改良血府逐瘀汤联合西医治疗慢性阻塞性肺疾病急性加重期并发肺动脉高压的临床研究

摘　　要：目的研究改良血府逐瘀汤联合传统西医治疗慢性阻塞性肺疾病急性加重期(AECOPD)并肺动脉高压的疗效、不良反应及其复发率。方法选择诊断为AECOPD并肺动脉高压患者50例,随机分两组,研究组:口服改良血府逐瘀汤联合传统西医(吸氧、抗炎、强心、利尿等)治疗,对照组:传统西医治疗,疗程均为30d。结果治疗后两组CRP、D-二聚体、ET-1、mPAP均明显降低,NO、6MWT两组均明显升高,两组比较有明显的统计学意义,P<0.05;两组总有效率、复发率比较有明显的统计学意义,P<0.05。结论改良血府逐瘀汤联合传统西医治疗慢性阻塞性肺疾病急性加重期并肺动脉高压疗效高,不良反应少,复发率低,可在社区推广。Objective To study the curative effect, untoward effect and recurrence rate in treatment with improved Xuefu Zhuyu Decoction combined traditional western medicine in AECOPD with Pulmonary Hypertension. Method To choose 50 patients which were diagnosed AECOPD with Pulmonary Hypertension, and divided into two groups randomly.Research Group： take improved Xuefu Zhuyu Decoction orally combined traditional western medicine （oxygen uptake, anti-inflammation, strong heart, diuresis and so on. Contral Group： treat with traditional western medicine. The course of the treatment was 30 days. Result In two groups, CRP, D-dimer, ET-1 and mPAP decreased, NO and 6MWT increased significantly. The differences between the two groups were significantly, P〈 0.05. There were obvious statistical significance in effective rate and recurrence rate in two groups, P〈 0.05. Conclusion There are definite curative effect, less adverse reaction and low recurrence rate by the treatment with improved Xuefu Zhuyu Decoction combined traditional western medicine in acute exacerbation of Chronic obstructive pulmonary diseases with Pulmonary Hypertension. It is worth popularizing community.
