医院-社区-家庭一体化管理对冠心病患者心脏康复的效果研究

摘　　要：目的调查分析医院-社区-家庭一体化管理模式对冠心病患者院外早期心脏康复的效果。方法选取2010年1月至2012年1月我院心内科病房治疗出院的急性心血管事件患者151例作为研究对象,按随机数字表法分为一体化管理组和对照组进行分析。结果 151例患者中有140例坚持随访,随访率为92.6%,其中观察组72例,对照组68例,两组患者再入院率比较差异有统计学意义(P<0.05)。.结论医院-社区-家庭一体化管理模式是冠心病患者院外早期心脏康复的有效措施。Objective To investigate the effect of acute cardiovascular events analysis method of integrated management mode of family hospital community of early cardiac rehabilitation in patients with coronary heart disease. Method From 2010 January to 2012 January in our hospital department of cardiology hospital ward in 151 patients as the research object, randomly divided into management group and control group were analyzed. Results In 151 cases, 140 cases with follow-up, follow-up rate was 92.6%, of which 72 cases in observation group, 68 cases in the control group, two groups of patients with re admission rate had significant difference （P〈0.05）. Conclusion The integrated management mode of family hospital-Communityare the effective measures for early stage patients with coronary heart disease outside.
