苏州地区不同级别医院急诊专科医生掌握核心急救技术程度的研究

摘　　要：目的 评价苏州地区二级以上医院急诊专科医生核心急救技术掌握程度,为今后急救技能培训项目的 确定提供依据.方法 通过全市举办急救技能竞赛的方式,每家医院分层随机抽取2名医生,参加心肺复苏（CPR）、心脏电除颤（D）、面罩加压给氧（OTFM）、气管插管（TI）、呼吸机初始参数设定（ISY）,以及颈椎固定（PCG）等6项技能测试.考核成绩〈60分为差,60~80分为良好,〉80分为优秀.依照急救技术的种类将竞赛成绩分成6组,比较每项技术的得分情况,并分析各项技术成绩〈60分人员的特点.结果 共有24家医院48名医生参加技能考核,每人均参加6项技能考核,技能考核人次为288人次;其中二级医院15家,三级医院9家,48名医生中除3人为非急诊科医生外,其他45人均为急诊专科医生.CPR与D成绩优秀的比例分别为73.3%和77.8%;ISV优秀者为71.1%;OTFM与TI成绩优秀者则分别为55.5%和46.7%;PCG成绩优秀者仅占22.2%,且明显低于其他组 （P〈0.05）.PCG组的平均成绩[（68.0±14.9）分,P〈0.05]和得分〈60分的人数均与其他组比较差异有统计学意义（P〈0.05）.各种技能得分〈60人员在学历、所在医院级别和急诊从业时间方面比较差异无统计学意义.结论 苏州地区二级以上医院急诊专科医生气管插管与颈椎固定技术掌握程度不高,应将这两项核心急救技术作为强化培训内容.Objective The study was made to investigate the current performance of the emergency physicians in the basic life support skills in second and third - level hospitals of Suzhou, in order to provide fundamental data for the future training programs. Methods The results of the competition of basic life support skills, which was organized by the Direction of Public Health of Suzhou, were collected and analyzed. The participants came from all hospitals of second and third levels in the area of Suzhou. Actually only two physicians from each hospital were randomly selected for the competition. They participated to compete in six skills including cardiopulmonary resuscitation （CPR）, defibrillation（ D）, oxygen therapy with face- mask（ OTFM）, tracheal intubation（TI）, initial setting of ventilator（ISV） and placement of cervical gear（PCG）. The competition scores were classified into 3 levels ： less than 60 points （ 〈 60） as poor score ; between 60 and 80 points （60 - 80） as good; more than 80 points （ 〉 80） as excellent. According to the six different skills, the competition results were correspondingly divided into six groups. We analyzed the scores for each skill and tried to find out the characteristics of the emergency physicians whose scores were less than 〈 60 points. Results Total of 48 physicians （of whom 45 emergency physicians and only 3 non - emergency physicians） from 24 hospitals （ 15 hospitals of second level and 9 hospitals of third - level） participated the competition. The competition results of 45 emergency physicians were analyzed in the present study. 23 physicians had medium or senior titles with more than 5 years work experience. The rates of excellent level of CPR, D, ISV, OTFM and tracheal intubation （TI） were 73. 3%, 77. 8%, 71. 1%, 55. 5% and 46. 7% respectively. However, the rate of excellent level for PCG was only 22.2% , which was significantly lower than other groups （ P 〈 0.05 ）. Moreover, the average score of PCG was （68.0 ＋ 14
