腹腔镜下外阴癌腹股沟淋巴结清扫术的临床应用研究

摘　　要：目的探讨外阴癌患者行腔镜下腹股沟淋巴清扫术的安全性和可行性。方法使用普通腹腔镜手术的设备和器械,对收治的8例外阴浸润癌患者行腔镜下腹股沟淋巴清扫术13侧,回顾性分析13侧腹股沟淋巴结清扫术的临床资料,分析手术时间、出血量、每侧淋巴结数目及并发症等。结果 8例患者均顺利完成腹腔镜下腹股沟淋巴清扫术,手术时间平均（85±31）min（45~120 min）、术中出血平均（22±9）mL（10~40 mL）、淋巴结数平均为（11±5）个（6~16个）。术后平均住院天数（7±2）d（5~12 d）。所有患者均未发生严重术中术后并发症。结论外阴癌患者施行腔镜下腹股沟淋巴清扫术安全可行,可有效降低术后并发症,是微创治疗外阴癌的一个有效方法,但其远期疗效有待进一步研究。【Objective】To investigate the feasibility and safety of laparoscopic excision in inguinal lymph nodes for vulvar cancer.【Methods】Eight cases of vulvar invasive carcinoma underwent laparoscopy inguinal lymph dissection by ordinary laparoscopy surgical equipments and instruments,five of which underwent bilateral dissection.Their clinical data were analyzed retrospectively and all indicators were compared,including operation time,blood loss,numbers of lymph node dissection and complications after surgery.【Result】All patients accepted bilateral laparoscopic inguinal lymphadenectomy successfully.Two cases of patients cleaned ipsilateral pelvic lymph nodes.Each side of the inguinal lymph node was removed.The medium operative time was（85±31） min（45~120 min）,the average bleed was（22 ±9） mL（10~40 mL）,and the mean number of nodes removed unilateral was（11 ±5）（6~16）.The average postoperative stay was（7±2） d（5~12 d）.No skin necrosis and wound infection were seen in inguinal region in any cases.【Conclusion】The laparoscopic excision of inguinal lymphanodes for vulvar cancer is a safe and feasible technique that appears to diminish the wound-related complications associated with the standard open approach.But its long curative effect is still to be researched.
