火针和刺络放血治疗寻常型斑块型银屑病90例疗效观察

摘　　要：目的观察火针和刺络放血治疗寻常型斑块型银屑病的临床疗效及安全性。方法将180例患者随机分为治疗组和对照组各90例,治疗组证属血燥者采用红斑刺络放血疗法,证属血瘀者采用局部火针疗法,每7d治疗1次;对照组给予他扎罗汀乳膏外搽,每晚1次,两组疗程均为8周。观察两组患者临床疗效及不良反应,并于治疗前和治疗后第2、4、8周采用银屑病皮损面积严重指数(PASI)评分标准对靶损害皮损面积、肥厚程度(浸润)、红斑及鳞屑情况进行评分。结果两组患者治疗后2周与治疗前PASI评分比较差异无统计学意义(P>0.05);两组治疗后4周、8周与治疗前比较,PASI评分均明显改善(P<0.05)。治疗组治疗4周、8周PASI评分与对照组同时间比较,差异均有统计学意义(P<0.01)。治疗组临床疗效总有效率(87.50%)明显优于对照组(81.18%),差异有统计学意义(P<0.05)。两组患者均无严重不良反应发生。结论火针和刺络放血治疗寻常型斑块型银屑病疗效显著,不良反应少,可作为有效治疗寻常型斑块型银屑病的针灸优选方法。Objective To observe the efficacy and safety of special needling such as fire needling, pricking and bloodletting for psoriasis vulgaris plaque. Methods Totally 180 patients were randomized into the treatment group and control group, with 90 in each. Fire needling or pricking and bloodletting was used in the treatment group according to the pattern once every 7 days. Tazarotene cream was applied externally in the control group once nightly. The treatment lasted for 8 weeks. The therapeutic effect and adverse reaction was observed in both groups. The lesion area, degree of hypertrophy （infiltration）, erythema and scaling was scored with Psoriasis Area and Severity Index （PASI） before and after treatment. Results There was no significant difference between before and after 2-week treatment in PASI score in both groups （P〉0.05）. The PASI scores were significantly improved in both groups after 4 and 8 weeks of treatment （P〈0. 05）, especially in the treatment group （P〈0.05）. The total effective rate of the treatment group and control group was 87.50% and 81.18% respectively, with a significant difference between groups （P〈0.05）. There was no serious adverse event in both groups. Conclusion Special needling such as fire needling, pricking and bloodletting is effective for psoriasis vulgaris plaque with less adverse reaction. It can be used as a preferred process of acupuncture for psoriasis vulgaris plaque.
