摘  要:目的 观察新西兰大白兔肾脏在轻度和重度积水时对CO2气腹压的耐受能力.方法 套管法建立左侧轻、重度肾积水模型,56只雄性新西兰大白兔随机分为3组:无积水处理组(N组,n=16)、轻度积水组(M组,n=20)、重度积水组(S组,n=20).N组行6、9、12、15 mm Hg(1 mmHg=0.133 kPa),M、S组分别行0、6、9、12、15 mmHg气腹加压处理1h.术后24h切取左侧肾脏,检测超氧趋化酶(SOD)活力、丙二醛(MDA)含量和肾脏组织细胞凋亡指数(AI).结果 N组行气腹加压时各压力下SOD活力分别为:(17.46±1.17)、(17.40±0.83)、(16.92±1.53)、(12.91 ±1.30) U/mg,15 mmHg组SOD活力较6、9、12 mm Hg组明显降低(P<0.05);MDA含量分别为:(0.13±0.03)、(0.14±0.02)、(0.13±0.01)、(0.38±0.04) mmol/g;AI值分别为:(5.23 ±1.05)、(5.51±0.90)、(6.13 ±0.90)、(41.60±3.62)%,15 mmHg组MDA含量和AI值较6、9、12 mm Hg组明显增加;M组SOD活力分别为:(17.17±1.33)、(16.55 ± 1.53)、(16.27±2.02)、(15.98±1.10)、(11.84±0.86) U/mg;MDA含量分别为:(0.15 ±0.01)、(0.15±0.02)、(0.15 ±0.01)、(0.16±0.02)、(0.44±0.04) mmol/g;AI值分别为:(7.83±1.19)、(8.43±1.05)、(8.63±0.70)、(9.60±1.58)、(43.03±4.39)%,S组SOD活力分别为:(16.52 ±0.68)、(16.43±0.87)、(10.54 ±0.59)、(10.26±0.69)、(10.13±0.79)U/mg;MDA含量分别为:(0.18±0.01)、(0.18±0.03)、(0.44±0.06)、(0.46±0.05)、(0.47±0.04) mmol/g; AI值分别为:(10.53±2.05)、(11.68±1.69)、(44.73 ±3.46)、(47.33 ±6.22)、(48.28±2.85)%,M组和S组气腹加压分别到达15 mm Hg和9 mm Hg时SOD活力显著降低(P<0.05),MDA含量显著升高(P<0.05),AI值升高(P<0.05).结论 重度积水肾脏行气腹手术处理时,应尽量减小气腹压力以防止肾脏进一步损伤.Objective To observe the toleration to CO2 pneumoperitoneum pressure in mild and severe hydronephrosis of rabbits.Methods Fifty-six New Zealand White male rabbits were randomly allocated in three groups:normal (N,n =16),mild hydronephrosis (M,n =20) and severe hydronephrosis (S,n =20).The mild and severe hydronephrosis was induced by ureteral tube method.The rabbits in N group were treated with 6,9,12,and 15 mmHg (1 mmHg =0.133 kPa) CO2 pneumoperitoneum pressure,and those in M and S groups were treated with 0,6,9,12 and 15 mm Hg CO2 pneumoperitoneum pressure.The time of pneumoperitoneum pressure was 1 h.The kidneys were harvested for measurement of super oxide dismutase (SOD),malonaldehyde (MDA) and apoptosis index (AI) of tubular cells after 24 h.Results In N group,SOD content was (17.46±1.17),(17.40±0.83),(16.92±1.53) and (12.91 ± 1.30) U/mg,MDA content was (0.13 ±0.03),(0.14 ±0.02),(0.13 ±0.01) and (0.38 ± 0.04) mmol/g,AI was (5.23 ± 1.05),(5.51 ±0.90),(6.13 ±0.90)% and (41.60 ±3.62)% respectively.SOD content was significantly decreased,MDA and AI were significantly increased in 15 mmHg as compared with those in 6,9,12 mmHg (P < 0.05).In M group,SOD content was (17.17 ± 1.33),(16.55±1.53),(16.27 ±2.02),(15.98±1.10) and (11.84±0.86) U/mg,MDA content was (0.15 ±0.01),(0.15 ±0.02),(0.15 ±0.01),(0.16 ±0.02) and (0.44 ±0.04) mmol/g,AI was (7.83 ± 1.19),(8.43 ± 1.05),(8.63 ± 0.70),(9.60 ± 1.58) % and (43.03 ± 4.39) %,respectively.In S group,SOD content was (16.52 ± 0.68),(16.43 ±.0.87),(10.54 ± 0.59),(10.26 ±.0.69) and (10.13±0.79) U/mg,MDA content was (0.18 ±0.01),(0.18 ±0.03),(0.44 ±0.06),(0.46 ±0.0) and (0.47 ±0.04) mmol/g,AI was (10.53 ±2.05),(11.68 ±1.69),(44.73 ±3.46),(47.33 ±6.22)% and (48.28 ±2.85)%,respectively.In M and S groups,SOD content was significa
