摘  要:目的通过检测术中胃黏膜CO2分压、胃黏膜pH值的变化,观察电针对术中CO2气腹副反应干预的效果。方法收治妇科腹腔镜手术患者60例,行随机、单盲、对照的临床试验。患者分为两组,Ⅰ组30例,施以常规全身麻醉,Ⅱ组30例,在常规全身麻醉基础上,复合穴位电针,从进入手术室开始一直持续到手术结束。术中获取反映CO2气腹副反应和动脉血气分析的相关数据,以备评价所用。结果胃黏膜CO2分压、胃黏膜pH值的组间和组内数值变化均存在显著差异(P<0.01)。结论电针应用在全麻妇科腹腔镜手术中,能够保证胃黏膜的血氧供应,防止胃黏膜局部缺血或低灌注,维持胃黏膜屏障的生理功能,达到纠正CO2气腹副反应的临床效果。Objective To observe the effect of electroacupuncture in intervening the CO2-pneumoperitoneum during operation, by detecting the changes of gastric mucosal CO2 partial pressure and pH level. Method Sixty patients who were going to receive gynecologic laparoscopy were enrolled into this randomized single-blind controlled clinical trial. They were randomized into two groups. Thirty subjects in group I were given ordinary general anesthesia; the other 30 subjects were given electroacupuncture in addition to the general anesthesia from the beginning to the end of the operation. The relevant data for reflecting CO2-pneumoperitoneum and arterial blood gas analysis were determined for further evaluation. Result The intra-group and inter-group differences in the gastric mucosal CO2 partial pressure and pH level were all statistically significant (P〈0.01). Conclusion When electroacupuncture is applied to the gynecologic laparoscopy, it can guarantee the oxygen supply to the gastric mucosa, prevent the topical ischemia or hypoperfusion, maintain the physiologic function of gastric mucosal barrier, and correct the CO2-pneumoperitoneum.
