54例主动脉缩窄的外科治疗

摘　　要：我院 1984年 6月至 1998年 6月 ,共收治 54例主动脉缩窄病人。男性 2 7例 ,女性 2 7例 ,平均年龄 15.8岁 ( 5个月～ 39岁 )。其中婴儿型 18例 ,成人型 36例 ,合并动脉导管未闭 33例 ,室间隔缺损 2 0例 ,房间隔缺损 4例 ,主动脉瓣二瓣化畸形 4例 ,主动脉瓣下狭窄 3例 ,二尖瓣狭窄 2例 ,二尖瓣脱垂 1例。其中 39例进行狭窄处补片扩大成形术 ,5例进行狭窄处切除主动脉端端吻合术 ,3例进行了狭窄处切除人工血管移植术 ,4例进行了人工血管转流手术 ,3例用主肺动脉内隧道的方法 ,实现了升主动脉 降主动脉的连接 ,同时行室缺修补。手术死亡 1例 ( 1.85% )。远期随访死亡 3例 ( 5.6% ) 。This paper presents the experience in surgical treatment of coarctation of the aorta form June 1984 to June 1998.This series of patients consists of 27 males and 27 females with a mean age of 15.8(5 months～39years)years old.54 patients included 18 cases of infantile type,36 cases of adult type;the associated intracardiac anomalies included 33 cases of patent ductus arteriosus,20 cases of ventricular septal defects,4 cases of atrial septal defects,4 cases of bicuspid aortic valve,3 cases of subvalve aortic stenosis,2 cases of mitral valve stenosis,1 case of mitral valve prolapse,The surgical techniques adopted 1)aortoplasty with patch graft in 39 cases(72.2%),2)stenosis resection with end to end anastomosis in 5 cases(9.3%),3)resection with artificial graft replacement in 3 cases(5.6%),4)bypass graft in 4 cases(7.4%),5)ascending aorta patent ductus arteriosus inner tunnel bypass graft with VSD repaire in 3 cases(5.6%).The operative mortality rate was 1.85%(1 case).Postoperative death was 5.6%(3 cases)during the first year.Follow up of a mean 2.64 years shows 50 patients doing well.
