规范化管理前后高血压患者医疗费用比较

摘　　要：目的探讨开展社区高血压病例管理适宜技术对社区高血压控制效果及其经济学意义。方法于2012年1—12月,采用多阶段抽样方法随机抽取西安交通大学社区卫生服务站管理高血压患者500例作为综合管理组;同期在西安交通大学医院内科门诊就诊的高血压患者中随机抽取500例作为门诊组。综合管理组以《社区卫生服务适宜技术手册——社区高血压病例管理》为指导,进行规范化管理;门诊组采用常规管理。比较两组高血压患者的医疗费用情况。结果综合管理组治疗率为93.32%、控制率为66.14%,分别高于管理前的39.70%和13.20%。门诊组管理后治疗率为67.55%,控制率为40.46%,高于管理前的38.91%和13.81%,管理前后比较,差异均有统计学意义(P<0.05)。结论对高血压患者进行规范化管理是控制高血压的有效手段。Objective To discuss the control effect and economic significance of appropriate technology in developing community hypertension management. Methods From January to December in 2012,muhistage sampling method was adopted to select 500 cases of hypertension from community health service station of Xi＇ an Jiaotong University randomly as integrated management group ; simuhaneously, 500 cases of hypertension were selected randomly from internal medicine department of Xi＇an Jiaotong University as outpatient group. Integrated management group was conducted with standardized management and guided by ＂Appropriately Technological Manual of Community Health Services--the Community Hypertension Case Management＂ ;while outpatient group employed normal management. Medical expenses of the hypertensive patients in the two groups were compared. Results The treatment rate was 93.32% and control rates was 66.14% in the integrated management group,which were higher than those before the management （39.70% and 13.20% respectively）. The treatment rate was 67.55% and the control rate was 40.46% in outpatient group,which were higher than those before the management（38.91% and 13.81% respectively）. Difference between before and after management had statistical significance （P〈0.05）. Conclusion It is an effective method to conduct stan- dardized management to control hypertension for hypertensive patients.
