摘 要:目的:探讨尿miRNA-192、miRNA-29c在芪蛭肾综颗粒治疗难治性肾病综合征(RNS)的机制。方法:将RNS患者44例随机分为实验组(芪蛭肾综颗粒加基础治疗)23例及对照组(黄葵胶囊加基础治疗)21例,经3月治疗后,观察尿miRNA-192、miRNA-29c与24h尿蛋白定量、总胆固醇(TC)、载脂蛋白(α)[LP(α)]、白蛋白(Alb)等指标的相关性,治疗前后尿miRNA-192、miRNA-29c的变化。结果:2组总有效率比较,差异有显著性意义(P<0.05)。尿miRNA-29c与LP(α)呈负相关,与Alb呈正相关,差异均有非常显著性意义(P<0.01)。尿miRNA-192差值与24h尿蛋白定量和TC呈正相关,差异有显著性或非常显著性意义(P<0.05,P<0.01)。2组治疗后与治疗前比较,miRNA-29c升高,miRNA-192降低,差异有显著性或非常显著性意义(P<0.01,P<0.05);治疗后2组miRNA-29c差值比较,差异有显著性意义(P<0.05)。结论:芪蛭肾综颗粒能抑制RNS的细胞外基质(ECM)增生,延缓肾小球硬化。尿miRNA-192、miRNA-29c可能为新型的非侵入型的检验指标。Objective: To study the relationship of urine miRNA-192 and miRNA-29c expression with the therapeutic mechanism of Qizhi Shenzong Granula for the treatment of refractory nephrotic syndrome(RNS). Methods: Forty-four RNS patients were divided into the test group and the control group, treatment lasting for 3 months. Both groups were given basic routine treatment, and additionally, the test group was given Qizhi Shenzong Granula and the control group was given Huangkui Capsules. The urine miRNA-192 and miRNA-29c expression levels were monitored before and after treatment, and their correlation with 24-hour urinary protein, total cholesterol(TC), apoprotein cx [LP(e)], albumin(AIb) was investigated. Results: The expression of urinary miRNA-29c was positively correlated with AIb and negatively correlated with LP(α)(P〈 0.01). The expression of urinary miRNA-192 was positively correlated with urinary 24-h urinary protein and TC(P〈 0.05, P〈 0.01). With comparison to those before treatment, miRNA-29c expression was increased, miRNA-192 was decreased in the two groups after the treatment, the differences being significant(P〈 0.01, P〈 0.05). The difference of miRNA-29c expression level was significant between the two groups after treatment (P 〈 0.05). Conclusion: Qizhi Shenzong Granula has an effect on inhibiting the proliferation of extracellular matrix(ECM) and delaying the glomerular sclerosis. Urinary miRNA-192 and miRNA-29c can be expected as new non-invasive test indexes for refractory nephrotic syndrome.
