阿托伐他汀对阻塞性睡眠呼吸暂停综合征患者臂-踝脉搏波传递速度的影响

摘　　要：目的评价阿托伐他汀对阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea syndrome,OSAS)患者臂-踝脉搏波传递速度(brachial-ankle pulse wave velocity,baPWV)的影响。方法利用多导睡眠图(PSG)入选165例OSAS患者,分为单纯OSAS组(B组)80例和OSAS合并高血压组(C组)85例。入选患者服用阿托伐他汀,在服药前和3个月后应用科林波形分析仪测量患者的baPWV。另外,选择同期81名健康体检者为正常对照组(A组)。结果B、C两组基础状态如腹围、颈围、体质量指数、呼吸暂停低通气指数(apnea hypopnea index,AHI)、低密度脂蛋白胆固醇、超敏C反应蛋白均明显高于A组,差异有统计学意义(P<0.05)。B、C两组治疗3个月后血清低密度脂蛋白胆固醇、超敏C反应蛋白浓度均明显降低,与治疗前比较,差异有统计学意义(P<0.001)。B、C两组baPWV较基础状态明显下降,差异有统计学意义[B组:(1 562±414)cm/s vs.(1 840±463)cm/s,P<0.001;C组:(1 596±433)cm/s vs.(1 866±428)cm/s,P<0.001]。结论阿托伐他汀不仅显著降低低密度脂蛋白胆固醇,抑制炎症反应,还对动脉弹性有明显的改善作用。Objectives To investigate the effects of atorvastatin on brachial-ankle pulse wave velocity （baPWV） in patients with obstructive sleep apnea syndrome（OSAS）.Methods Totally 165 patients with OSAS diagnosed by polysomnography （PSG） were administered atorvastatin for three months and divided in two groups （group B：80 cases without hypertension;group C：85 cases with hypertension）.BaPWV was measured by an automated device for brachial-ankle before and three months after treatment.Meanwhile,81 normal subjects （group A） were enrolled.Results At baseline,the abdominal circumference,neck circumference,body mass index （BMI）,apnea hypopnea index （AHI）,lowdensity lipoprotein （LDL） and high-sensitive C-reactive protein （hs-CRP） in both group B and C were significantly higher than those in group A （P〈0.05）.After three months＇ treatment,LDL and hs-CRP significantly decreased in group B and C （P〈0.001）.Compared with baseline,baPWV after treatment decreased significantly in group B and C [group B：（1 562±414） cm/s vs.（1 840±463） cm/s,P〈0.001; group C：（1 596±433） cm/s vs.（1 866±428）cm/s,P〈0.001].Conclusions Atorvastatin not only can decrease LDL level and inhibit hs-CRP in plasma efficiently,but also,to some extent,can improve the artery flexibility in patients with OSAS.
