摘 要:目的分析普米克令舒联合氨溴索雾化吸入治疗慢性阻塞性肺疾病(COPD)的临床疗效。方法将92例COPD患者随机分为观察组和对照组,每组46例。两组均接受吸氧、抗感染、支气管扩张等常规治疗,在此基础上,观察组雾化吸入普米克令舒联合氨溴索,对照组静脉推注甲强龙。治疗7天后,比较两组患者的临床疗效、肺功能相关指标及血气分析情况。结果观察组总有效率明显高于对照组,差异有统计学意义(P<0.01);两组治疗后动脉氧分压(PaO2)、第一秒用力呼气量(FEV1)和FEV1/用力肺活量(FVC)%均较治疗前明显升高,而动脉二氧化碳分压(PaCO2)明显降低(P<0.01);治疗后,观察组PaO2、FEV1及FEV1/FVC%均显著高于对照组,而PaCO2显著低于对照组,差异有统计学意义(P<0.01)。结论普米克令舒联合氨溴索雾化吸入能有效改善COPD患者临床症状,提高肺通气功能,纠正二氧化碳潴留及低氧症状,其疗效显著优于甲强龙。Objective To analyze the therapeutic effect of aerosol inhalation of pulmicort respules and ambroxol for treating chro- nic obstructive pulmonary disease (COPD). Methods Ninety - two patients with COPD were randomly divided into two groups,46 cases in each group. The patients in two groups were treated with routine treatment including oxygen uptake, anti - infection and bronchial dilation. Based on this, the observation group was given aerosol inhalation of pulmicort respules and ambroxol,while control group was injected methylprednisolone intravenously. 7days after treatment,the clinical effects, pulmonary function- related indexes and blood gas analysis in two groups were compared. Results The total effective rate of observation group was obviously higher than control group, with statistical significance (P 〈 O. 01 ). After treatment, PaO2, forced expiratory volume in one second ( FEVj ) and FEV1/forced vital capacity (FVC)% in two groups evidently went up compared with prior to treatment, while PaCO2 notably went- down ( P 〈0. O1 ). Besides ,PaO2 ,FEV1 and FEV1/FVC% in observation group were significantly higher than in control group,while lower than control group in PaCO2, and the difference had statistical significance ( P 〈 0. Ol ). Conclusion Aerosol inhalation of pulmicort respules and ambroxol can effectively ameliorate the clinical symptoms of patients with COPD,improve pulmonary ventila- tion, and correct carbon dioxide retention and hypoxia, with the therapeutic effect superior to methylprednisolone.
