三甲医院药师在社区开展药学服务工作模式的探讨

摘　　要：目的探索建立一条三甲医院药师更好的服务社区患者的模式。方法2011年3月至2012年9月北京安贞医院药剂师对205名社区居民进行药学服务需求调查，药物咨询，安全用药知识讲座，建立“知己患者药历”等方式探讨可行的工作模式。结果205名社区居民中，132人（64．4％）需要药师进行专业服务，164人（80．0％）信赖药师。社区咨询，解答215人次的专业药学问题，进行了5次安全用药知识讲座，建立了8份“知己患者药历”。结论三甲医院药师采取这些工作模式深入社区开展药学服务是可行的，对今后开展工作有借鉴意义。Objective To find a good pharmaceutical care of pharmacists in three-A hospitals in communi- ty. Methods From March 2011 to September 2012, pharmacists from Anzhen hospital did an investigation regard- ing community residents pharmaceutical care requirements, medication enquiry in community, gave drug safety knowledge lecture and established personal medication records. Results Among 205 persons, 132 persons （64.4%） needed pharmaceutical care and 164 persons （80.0%）trusted the pharmacist. Medication enquiry more than 215 times were made. Five lectures were given to the community residents and 8 medication records were built. Conclusion Pharmacists in three-A hospitals developing pharmaceutical care in community is possible and has sig- nificant reference for future work.
