摘 要:为明确Trietz韧带下离断不同空肠距离对2型糖尿病(Type 2 diabetes,T2DM)滇南小耳猪动物模型术后血糖的影响,将20只雌性滇南小耳猪随机分为正常对照组(NC组)、糖尿病对照组(DC组)、糖尿病手术1组(DO1组)和2组(DO2组),每组5只.DC、DO1及DO2组采用高糖高脂饮食加小剂量链尿佐菌素诱导建立T2DM动物模型.建模成功后(第8周末)DO1组胃转流术(Roux-en-Y gastric bypass,RYGB)(Treiz韧带下3m离断空肠),DO2组RYGB(Treiz韧带下6m离断空肠),监测各组术前、术后2、4及8周空腹血糖、血清胰岛素、胰高血糖素样肽-1(Glp-1)、葡萄糖依赖性促胰岛素释放肽(GIP),并比较抵抗指数(HOMA-IR)变化.与DC组相比,DO1、DO2组术后血清胰岛素和Glp-1浓度明显升高(P<0.05),胰岛素抵抗明显改善(P<0.05);DO1、DO2组空腹血糖和GIP浓度明显下降(P<0.05),DO2组血糖下降更明显(P<0.05).实验显示:Treiz韧带下更远距离离断空肠可获更好降糖效果.To study the effect of different jejunum amputation distance under Trietz ligament transaction on the blood glucose, 20 healthy female southern Yunnan miniature pigs are randomly divided into normal control group (NC group, n = 5 ) and diabetic control group (DC group, n = 5 ), diabetes surgery in group 1 (DO1 group, n = 5 ) and diabetes surgery in group 2 ( DO2 group, n = 5 ). DO1, DO2 and DC group miniature pigs are pro- vided with high sugar high fat diet and small doses of streptozotocin to induce and establish type 2 diabetic mod- els. When the models are stable (8th weeks ), the DO1 group has Roux-en-Y gastric bypass(RYGB)(jejunum Treiz ligament expansion set to 3m) , and the DO2 group RYGB ( the jejunum Treiz ligament expansion set to 6m). Fasting blood glucose, serum insulin enzyme-linked immunosorbent assay, pancreatic glucagon-like pep- tide - 1 ( Glp - 1 ), glucose-dependent insulin-releasing (GIP) and the insulin resistance index ( HOMA - IR) of all miniature pigs peptide are monitored before and postoperative 2th, 4th, 8th weeks. Compared with those of DC group, the concentrations of serum insulin and GLP - 1 in DO1 and DO2 groups are significantly increased( P 〈 0.05 ) and the insulin resistance is improved significantly ( P 〈 0.05 ), while the fasting blood glucose and GIP are significantly lower ( P 〈 0. 05 ). The blood glucose of DO2 group decreases even more significantly (P 〈 0.05). It is shown through the results that the more remote jejunum amputation distance under Treiz ligament obtains better postoperative hypoglycemic effect.
