摘　　要：目的 利用某三级医院2010～2011年住院病人资料分析急性心肌梗死患者住院费用及相关因素,为合理控制急性心肌梗死医疗费用提供依据.方法 收集2010～2011年潍坊市某三甲医院急性心肌梗死病例1060例,采用描述性统计方法研究住院费用的变化趋势和构成情况,采用相关分析、秩和检验和多重线性回归等方法研究患者性别、年龄、出院转归、住院天数等对住院费用的影响.结果 急性心肌梗死患者住院费用平均29 466.46元,中位数25284.06元,其中以治疗费最高,占55.02％,其次是药费占28.31％、检查费占15.12％,床位费仅占0.49％、其他费用占1.06％.2年间住院费用呈下降趋势.影响住院费用的主要因素是住院天数、出院转归、特级护理和输血情况等.结论 急性心肌梗死患者住院费用高,经济负担重.应实施临床路径管理,完善医疗保障制度,降低医疗费用.OBJECTIVE Based on the data of inpatients in a third class hospital from 2010 to 2011, to analyze the hospitalization cost and relative factors of acute myocardial infarction （AMI）, and to provide some bases for controlling the cost of acute myocardial infarction inpatients. METHODS We picked up the AMI inpatients＇ data from a third class hospital in Weifang city from 2010 to 2011. A total 1 060 AMI cases were registered. Trend and proportion of AMI inpatients＇ cost were analyzed with descriptive statistical analysis. Some influence factors such as gender, age, prognosis and days of inpatient were analyzed with correlation analysis, Rank sum test and multiple linear regression. RESULTS The average cost of inpa- tients was RMB 29 466.46 Yuan and P50 was RMB 25 284.06 Yuan. The highest cost was the therapeutic cost （55.02%） in all cost. Then the medicament was 28.31%, physical examination was 15.12%, ward was 0.49% and else was 1.06%. The cost was in a downtrend from 2010 to 2011. Mainly influence factors were hospital stay of inpatients, prognosis, intensive care and blood transfusion. CONCLUSION The cost of inpatients and the economic burden are high in AMI patients. We suggest that enforce the clinical path management and perfect the system of medical insurance for reducing medical cost.
