舌脉指标在心病、脑病、心脑合病痰瘀互结证中的分布研究

摘　　要：目的:探讨舌脉指标在不同疾病的痰瘀互结证中分布的异同,为异病同治提供临床证据支持。方法:运用临床流行病学横断面调查方法,纳入冠心病心绞痛(以下简称心病)痰瘀互结证患者105例,动脉粥样硬化性血栓性脑梗死(以下简称脑病)痰瘀互结证患者143例,冠心病心绞痛合并动脉粥样硬化性血栓性脑梗死(以下称心脑合病)痰瘀互结证患者160例,收集患者的一般资料、相关危险因素、四诊信息、证候诊断等。运用频数、卡方检验、关联规则等方法分析三病舌脉表现的异同。结果:三病出现频数最高的舌脉指标均为弦脉、滑脉、苔色白等。三病做卡方检验P>0.05的指标有弦脉、滑脉、苔色白等,P<0.05的指标有舌态歪斜、舌态强硬、舌质老等。经关联规则分析得出心病的常见舌脉组合为舌边有齿痕,舌紫黯、苔白腻或厚腻、脉弦滑或沉,脑病的常见舌脉组合为舌体胖,边有齿痕,舌下脉络青紫或迂曲、舌有斑点,苔白腻或厚腻,脉弦滑或沉滑,心脑合病的常见舌脉组合为舌体胖,边有齿痕,舌淡黯,舌下脉络青紫或迂曲,苔滑腻或厚腻、脉弦滑、沉滑或短脉。结论:在不同疾病的相同证型中,舌脉表现大体相同,微小差异。Objective： Differences and similarities of tongue and pulse indicators distribution in syndrome of turbid phlegm and blood stasis of three different diseases were investigated,in order to provide clinical basis for treating different diseases with the same therapeutic principle. Method： Clinical epidemiological cross - sectional research methods were used. Information of syndrome of turbid phlegm and blood stasis of 105 cases of angina pectoris, 143 cases of atheroscle- rotic thrombotic cerebral infarction and 160 cases of angina pectoris complicated with atherosclerotic thrombotic cerebral infarction （XNHB） were collected, including general condition, risk factors, syndrome diagnosis, etc. Differences and sim- ilarities of tongue and pulse indicators distribution in syndrome of turbid phlegm and blood stasis of three different disea- ses were analyzed, using calculated and described, chi - square test and association rules. Result： The most common tongue and pulse indicators in three different diseases were string pulse, smooth pulse, white tongue fur, etc. The chi - square test result between three groups showed that the indicators differences were statistically significant （ P≥ 0.05 ） ： string pulse, smooth pulse, white tongue fur, etc ; the indicators differences were statistically insignificant （ P 〈 0.05 ） ： dis- tortion of tongue, rigidity tongue, old tongue, etc. The association rules result showed that the common tongue and pulse indicators combinations of angina pectoris were teeth print on tongue margin, purplish tongue body,white greasy tongue fur or thick greasy tongue fur, taut and slippery pulse or sinking, the common tongue and pulse indicators combinations of ath- erosclerotic thrombotic cerebral infarction were swollen tongue body, teeth print on tongue margin, cyanosis or circuity cho- roid under tongue, purple spots on the tongue, white greasy tongue fur or thick greasy tongue fur, taut and slippery pulse or sinking slip pulse ; the common tongue and pulse indicators
