摘　　要：目的 探讨经食管实时三维超声心动图（RT-3D TEE）测量心房颤动（房颤）患者肺静脉开口内径的可行性及准确性.方法 107例入院行射频消融术的房颤患者,术前使用RT-3D TEE观察肺静脉开口并测量其内径（PVD）,并与多排螺旋CT（MDCT）进行对比.结果 左上肺静脉（LSPV）和左下肺静脉（LIPV）超声显示率为100％,右上肺静脉（RSPV）为92.52％（99/107）,右下肺静脉（RIPV）为87.85％（94/107）.两组间LSPV[（21.96±4.71） mm对（23.78±3.35） mm]、LIPV[（16.72±4.56） mm对（17.96±2.99） mm]、RSPV[（22.62±4.33） mm对（23.88±3.87） mm]开口的测量值差异无统计学意义（P＞0.05）,而RIPV的开口超声测量值明显小于MDCT测量值[（15.43±5.69）mm对（17.89±3.94） mm],差异有统计学意义（P＜0.05）.左侧肺静脉开口测量值在两组间一致性较好,且上肺静脉比下肺静脉组间一致性高.结论 RT-3D TEE可用于显示肺静脉开口,有望在未来替代MDCT.Objective To investigate the feasibility and accuracy of real time three dimensional transesophageal echocardiography（RT-3D TEE）versus multi-detector row spiral CT（MDCT） for the evaluation of of pulmonary vein ostial morphology in patients with atrial fibrillation.Methods In 107 patients undergoing radiofrequency ablation,pre-operative pulmonary vein ostial diameter（PVD） measurements obtained by RT-3D TEE and MDCT were compared.Results The display ratio of the left pulmonary vein （LSPV,LIPV） was 100%,of the right upper pulmonary vein（RSPV） was 92.52% （99/107）and of the right lower pulmonary vein （RIPV） was 87.85 % （94/107）.There was no statistical difference between the PVD of LSPV [（21.96 ± 4.71）mm vs（23.78±3.35） mm],LIPV[（16.72±4.56）mm vs（17.96±2.99） mm] and RSPV[（22.62±4.33）mm vs （23.88±3.87） mm] obtained by RT-3DTEE and MDCT（P〉0.05）.While the PVD of RIPV obtained by RT-3D TEE was smaller than that obtained by MDCT [（15.43±5.69） mm vs（17.89±3.94） mm;P〈0.05].PVD of the left pulmonary veins was significantly correlated with that tneasured by MDCT,meanwhile the correlation of the upper pulmonary veins was better than the lower between the two groups.Conclusion RT-3D TEE is useful in the evaluation of of pulmonary vein ostial morphology.It may be used as an alternative to MDCT in the future.
