经脐单孔针式腹腔镜胆囊切除术1780例体会

摘　　要：目的：探讨经脐单孔针武腹腔镜胆囊切除术（TusPLc）的安全性和可行性。方法：回顾分析解放军第451医院内镜外科二区2010年～2012年应用针式腹腔镜1780例临床资料。结果：本组1780，24例因胆囊三角区粘连、炎症、解剖变异和创面大需放置引流中转3孔或4孔LC，手术平均时间35．5min，出血量平均17．2mL，术后均无并发症。脐部切口小而隐藏脐凹，无可视瘢痕。结论：术前和术中慎重把握手术适应证，经脐单孔腹腔镜胆囊切除术是安全的，具有明显的美容优点。Objective to study the umbilical single-hole pin type （TUSPLC） the safety and feasibility of laparoscopic cholecystectomy. Methods retro-spective analysis the people＇s liberation army 451th hospital endoscopic surgical area of 2010-2012,application of needle laparoscopy, 1780 cases of clinical data. Results of 1780,24 patients with gallbladder triangle anatomical variations and the adhesion,inflammation,wounds need to set drainage of large bore transfer 3 or 4 LC, average operation time was 35.5 min, blood loss, on average, 17.2 mL, there were no postoperative complications. Navel incision small and hidden umbilical concave, no apparent sear. Conclusion preoperative and intraoperative carefully grasp the operation indication, the umbilical single-arch laparoscopic cholecystectomy is safe and has obvious advantages of beauty.
