摘 要:目的探讨现代泌尿外科微创治疗输尿管结石的最优治疗方案。方法对需要手术治疗的172例输尿管结石患者,依据不同结石位置,选择微通道经皮肾镜碎石术(MPCNL)、腹腔镜输尿管切开取石术(LUL)及输尿管镜钬激光碎石术(URL)治疗,并针对输尿管上段结石治疗成功率与相关文献采用输尿管镜碎石的成功率进行比较。结果治疗输尿管上段结石视结石部位选用MPCNL或LUL平均成功率98.0%,较其他文献报道采用输尿管镜碎石的成功率明显提高,URL治疗输尿管中下段结石成功率93.0%,达到国内同级水平。结论需手术治疗的输尿管结石中,盂管交界段建议采用MPCNL,肾下极段采用LUL,输尿管中下段结石采用URL。Objective To explore the optimum therapeutic plan of modem minimally invasive technology of urology in treating ureteral calculus. Methods Depending on the location of the calculus, 172 patients with ureteral calculus,who needed operative treatment, were with minimally invasive percutaneous nephrolithotomy (MPCNL), laparoscopic ureterolithotomy (LUL) or ureteroscopy holmium laser lithotripsy (URL). The success rate of calculus treatment on upper ureter and success rate of ureteroscopy lithotripsy in related literatures were compared. Results The average success rate of calculus on upper ureter treated by MPCNL or LUL depending on the location of calculus was 98.0% ,which increased obviously compared with that from the other literature reported;the success rate of URL for the treatment of calculus on middle and lower ureter was 93.0%,which reached the domestic level. Conclusion Among the ureteral calculi required surgery, MPCNL is recommended to use in uretero- pelvic junction section, LUL in lower pole renal segment and URL in middle and lower ureter.
