摘  要:目的探讨自体颗粒状松质骨打压植骨联合骨小梁金属髋臼杯在骨质疏松患者全髋关节置换术(THA)中的疗效。方法2009年12月~2011年5月,我院42例骨质疏松性患者应用自体颗粒状松质骨打压植骨联合骨小梁金属髋臼杯进行了THA。采用Harris评分评价手术前后髋关节功能、Conn影像学评价标准观察骨长人情况,Delee-Charnley三区法测量臼杯与移植骨间的界面宽度。结果所有患者均获得随访,平均随访时间26个月,Harris评分术前平均(35.4±12.7)分,到随访结束时提高到平均(78.6±6.9)分;所有患者影像学x线表现稳定,植入骨生长良好,假体位置良好,髋臼假体周围未见透亮影;2例患者术后大腿远端有轻度疼痛,无翻修手术病例。结论骨质疏松患者行自体颗粒状松质骨打压植骨联合骨小梁金属髋臼杯的THA疗效令人满意。Objective The aim of this study was to evaluate the efficacy of the total hip arthroplasty with the autologous particulate cancellous bone grafting and bone trabecular metal acetabular cup in osteoporosis patients. Methods A total of 42 osteoporosis patients were used autologous particulate cancellous bone grafting and bone trabecular metal acetabular cup in total hip arthroplasty. According to the Harris score,Conn's imaging evaluation of bone ingrowth,Delee's measuring three districts with the interface between the bone grafting width and thigh pain, we evaluated the operation efficiency in these osteoporosis patients. Results 42 patients were followed up for 20-36 months,average of 26 months,the results were satisfactory,the mean preoperative Harris hip score was (35.4± 12. 7) points,and the end of the follow-up mean was (78. 6 ± 6. 9) points. Imaging findings in patients showed that stable implant bone grew well,prosthesis was in good position and there was no acetabular periprosthetic translucent shadow. Two patients had mild thigh pain and no patient underwent revision surgery. Conclusion The efficiency of the total hip arthroplasty with autologous particulate cancellous bone grafting and bone trabecular metal acetabular cup in osteoporosis patients is satisfactory.
