实测经络针灸疗法治疗糖调节受损的研究

摘　　要：目的:验证实测经络针灸疗法治疗糖调节受损的临床疗效。方法:本研究纳入糖调节受损患者59例。分为对照组31例及实测经络针灸治疗组28例。实测经络针灸治疗组包括足三里(ST36)组、足三里(ST36)+手三里(LI10)组、足三里(ST36)+三阴交(SP6)3个亚组,运用实测经络针灸治疗1月后,比较各组口服葡萄糖耐量试验(OGTT 0min、30 min、60 min、120 min、180 min)的血糖及平均血糖,血糖曲线下面积(AUCg)、糖化血红蛋白(HbA1c),胰岛素水平,胰岛素曲线下面积(AUCI),胰岛素抵抗指数(HOMA-IR)、胰岛素敏感性IS指数(Matsuda index)、β细胞功能指数(△I30/△G30IR、MBCI、I相Stumvoll index)。结果:足三里+三阴交组治疗前OGTT60 min血糖(11.02±0.83)-1-1mmol/L,120 min血糖(9.06±0.85)mmol/L,平均血糖(8.10±0.62)mmol/L,AUCg(26.28±1.89)mmol·L-1·h-1。经过1月的实测经络针灸治疗干预后发现,足三里+三阴交组治疗后OGTT60min血糖(9.77±1.01)mmol/L,120 min血糖(6.98±-1-11.85)mmol/L,,平均血糖(7.32±0.94)mmol/L,AUCg(22.92±3.32)mmol·L-1·h-1,均较治疗前明显降低(P<0.05),有显著差异。足三里+三阴交针灸组治疗后与其它三组治疗后相比,其OGTT30 min、60 min或120 min的血糖及平均血糖,AUCg均有明显下降(P<0.05),有显著差异。结论:实测经络针灸疗法治疗糖调节受损的患者,选择足三里+三阴交针灸治疗对糖尿病前期人群的餐后高血糖有明显改善,对于延缓2型糖尿病的发生及降低心血管事件的发生和预防严重糖尿病微血管并发症的发生有着深远的意义。Objective To validate the curative effect of experimental acupuncture-meridian therapy for patients with impaired glucoregulation. Methods 59 patients with impaired glucoregulation were selected. There were 31 patients in the control group with general advices only but without positive interventions. There were 28 patients in experimental group of experimental acupuncture-meridian treatment for one month. The experimental group included three sub-groups： Zusanli （ST36） group, Zusanli（ST36） ＋ Shousanli（LI10） group, Zusanli （ST36） ＋ Sanyinjiao（SP6） group. We measured and compared blood pressure, body weight, body mass index （BMI）, waist-height ratio （WSR）, oral glucose tolerance test （OGTT） （0 minute, 30 minutes, 60 minutes, 120 minutes, 180 minutes）, the average glucose, area under the curve of blood glucose （AUCg）, glycosylated hemoglobin （HbAlc）, area under the curve of insulin （AUCI）, insulin resistance index（HOMA-IR）, IS index （Matsuda index）, and beta cell function index（AI30/AG30IR, MBCI, Iphase Stumvoll index）in all groups. Results The values before treatment of Zusanli （ST36） ＋ Sanyinjiao （SP6） group were OGTT （60 minutes） （11.02± 0.83） mmol/L, OGTT （120 minutes） （9.06 ±0.85） mmol/L, the average glucose （8.10±0.62） mmol/L, and AUCg （26.28± 1.89） mmol·L-1· h-1. The values after the experimental acupuncture-meridian treatment for on e month in Zusanli （S T36） ＋ Sanyinjiao （SP6） group were OGTT （60 minutes） （9.77±1.01）mmol/L, OGTT （120 min） （6.98±1.85） mmol/L, the average glucose （7.32±0.94）mmol/L, and AUCg （22.92± 3.32）mmol. L-1. h-l, which were decreased with significant differences than the values before treatment （P 〈 0.05）, or in those other three groups of patients （P 〈 0.05）.
