摘　　要：目的探讨两种方法预防产后出血的效果。方法选择2011年1—12月自然分娩的产妇600例,随机分为A组、B组各300例,A组在胎儿娩出后,立即于宫颈注射益母草注射液2 ml+缩宫素20 U。B组在胎儿娩出后,立即宫颈注射缩宫素20 U,同时卡孕栓1 mg舌下含服。两组同时辅以手法按摩子宫(胎儿娩出后,助手将一只手呈半握拳状在子宫底部适当用力按压、按揉子宫,以刺激、促进子宫收缩)至胎盘娩出3 min。记录第三产程时间,测量产后2、24 h出血量。同时记录产妇用药前后血压、心率的变化情况及各种异常表现和不适。计量资料采用t检验,两组不良反应比较采用秩和检验,P<0.05为差异有统计学意义。结果 A组产后2、24 h出血量[(155±38)、(167±42)ml]与B组[(161±34)、(218±37)ml]比较差异均有统计学意义(均P<0.05)。两组不良反应比较差异有统计学意义(P<0.05)。结论益母草注射液与宫缩素联合应用可明显减少产后出血,效果优于卡孕栓联合缩宫素。Objective To discuss the efficacy of two different methods in preventing postpartum hemorrhage. Methods A total of 600 parturient women with natural childbirth from January to December in 2011 were selected and divided into group A and group B, 300 cases for each group. Leonurns beterophyUus injection （2 ml）with oxytocin （20 U）was injected into the neck of uterus in group A immediately after the fetus was expulsed. Oxytocin （20 U） was injected into the neck of ute- rus in group B after expulsion of the fetus as well as Carprost 1 mg for sublingual administration. After delivery, uterus massage was given to both groups until 3 rain after the expulsion of placenta, during which the assistant performed pressing and rubbing to the uterus with one half - closed hand on the bottom of uterus to stimulate and promote uterine contractions. The duration of third stage was recorded, the amount of postpartum hemorrhage within 2 and 24 hours was measured, blood pressure, heart rate and uncomfortable feelings were observed before and after treatment. The t - test was used for measure- ment data, rank sum test for comparison of adverse reactions between two groups, the result of P 〈 0.05 was considered to be statistically significant. Results There was significant difference between group A and group B in the amount of post - operative bleeding within 2 and 24 hours [group A：（155±38） ,（167±42）m1; group B：（161 ±34）, （218±37）ml] （P 〈 0.05 ）. Significant difference was found between two groups in adverse reaction （ P 〈 0.05 ）. Conclusions Leonurus beterophyllns injection combined with oxytocin can significantly reduce postpartum bleeding, which is superior to Carbu- lets.
