摘 要:目的调查分析多药耐药克雷伯菌获得性耐药相关基因与可移动遗传元件遗传标记的相关性。方法 20株多药耐药克雷伯菌采用聚合酶链反应(PCR)法检测β-内酰胺类、氨基糖苷类、喹诺酮类、利福平类药物、消毒剂获得性耐药基因和整合子、转座子、插入序列、质粒等可移动遗传元件标记基因,用指标聚类法分析获得性耐药相关基因和可移动遗传元件遗传标记的相关性。结果 20株多药耐药克雷伯菌对15种抗菌药物的耐药率为35.0%~100.0%,检出7种β-内酰胺编码基因、4种氨基糖苷类修饰酶编码基因、2种喹诺酮类药物获得性耐药基因、1种利福平类药物获得性耐药基因、1种消毒剂获得性耐药基因、9种可移动遗传元件;指标聚类分析显示KPC与ISKpn6、SHV、qacE△1与intⅠ1、tnp315、IS26高度相关联。结论调查多药耐药克雷伯菌携带获得性耐药相关基因可导致对相关抗菌药物耐药,指标聚类分析提示获得性耐药基因和可移动遗传元件密切相关。OBJECTIVE To investigate the correlation between the acquired resistance-related genes and the markers of mobile genetic elements in multidrug-resistant Klebsiella. METHODS The acquired resistance genes to β- lactams, aminoglycosides, quinolones, rifampicin, disinfectants and genetic markers of mobile genetic elements including conjugal plasmids, transposons, insertion sequences, and integrons in 20 strains of multidrug-resistant Klebslella were detected by polymerase-chain-reaction(PCR) then the index cluster analysis was performed to analyze the relationship between the acquired resistance genes and the markers of mobile genetic elements. RESULTS The drug resistance rates of the 20 strains of multidrug-resistant Klebsiella to 15 antimicrobial drugs varied between 35. 0K and 100. 0%, and 7 acquired ^-lactam-resistance genes, 4 acquired aminoglycoside- resistance genes, 2 acquired quinolone-resistance genes, 1 acquired rifampicin-resistance gene, 1 acquired disinfectant- resistance gene, and 9 mobile genetic elements have been detected. Index cluster analysis showed that the resistance genes(KPC, SHV, qacE△1) were highly correlated with mobile genetic elements(ISKpn6, int I 1, tnp315, IS26). CONCLUSION In this group of Klebsiella strains, the acquired resistance genes may result in the drug resistance to related antibiotics, and the index cluster analysis indicates that the acquired resistance genes are closely related to the mobile genetic elements.
