不同静脉入路植入床旁临时心脏起搏器对比研究

摘　　要：目的比较不同手术路径植入床旁临时心脏起搏器的效果。方法对2008年9月至2012年9月来巴彦淖尔市医院就诊的37例心律失常患者进行不同静脉入路心脏临时起搏器植入的对比研究,均进行床旁心电监护,其中21例患者经左锁骨下静脉入路、16例患者经右股静脉入路植入临时心脏起搏器。比较两组患者静脉穿刺时间、开始植入电极到成功起搏时间、起搏阈值、感知、留管时间以及并发症情况。结果 37例患者均成功起搏,两组患者在穿刺时间、起搏阈值、感知、留管时间等方面比较差异无统计学意义(P>0.05),开始植入电极到成功起搏右心室时间、右心室成功起搏植入电极次数的比较,差异均有统计学意义(P<0.05)。结论经左锁骨下静脉植入较股静脉植入心脏起搏器起搏迅速,更适合无X线指导床旁心脏临时起搏。Objective To compare the effect of bedside temporary cardiac pacemaker installation with different surgical paths. Methods Thirty-seven arrythmia patients from Bayannaoer Hospital during September 2008 and September 2012 installed with temporary cardiac pacemaker by different surgical paths were compared, all of which were given bedside ECG, including 21 patien[s with left subclavian vein approach and 16 patients with right femoral vein approach. Venipuneture time, time of starting to install the electrode to successful pace-making, threshold values, perception time of pace-making, time of remaining duet and compli- cations of the two groups were compared. Results All cases succeeded. Puncture time, threshold values of pace-making, perception time,time of remaining duct of the two groups had no statistically significant differ- ence（ P 〉 0.05 ）. Time of starting to install the electrode to successful pace-making, and times of successful install electrodes to right ventrieular were different between the two groups with statistically significant differenee（ P 〈 0.05）. Conclusion Left subclavian vein approaeh is faster than right femoral vein approach and mnre suitable for temporary bedside cardiac pacing without x-ray guidance.
