宫腔镜治疗宫角妊娠的临床疗效观察

摘　　要：目的分析宫腔镜治疗早期宫角妊娠的临床效果及安全性。方法将宝鸡市中心医院2012年1月至2013年1月收治的56例早期宫角妊娠患者依据随机数字表法分为两组:宫腔镜治疗组(36例)采用宫腔镜治疗,常规治疗组(20例)采用B超下常规吸宫手术,观察两组患者的术中出血量、手术成功率、术后β人绒毛膜促性腺激素(β-HCG)下降时间、月经恢复情况及再次妊娠结局。结果宫腔镜治疗组手术成功率为100%,常规治疗组手术成功率为80%,两组比较差异有统计学意义(P<0.05);宫腔镜治疗组术中出血量少于常规治疗组,术后恢复月经时间及β-HCG下降时间也短于常规治疗组,两组比较差异有统计学意义(P<0.05);宫腔镜治疗组妊娠率为69.44%,常规治疗组妊娠率为40%,两组比较差异有统计学意义(P<0.05)。结论对未宫角妊娠,宫腔镜下可进一步明确诊断,同时可发现其他宫腔内疾病。宫腔镜治疗宫角妊娠较传统吸宫手术安全、微创,且患者出血少,疗效满意。Objective To analyze the clinical effects and safety of hysteroscopy treatment tbr cornual pregnancy. Methods Fifty-six cases of early uterine horn pregnancy admitted to Baoji Central Hospital from January 2012 to January 2013 were randomly divided into two groups： hysteroscopic treatment group （36 cases） using hysteroscopie treatment, the conventional treatment group （ 20 cases） using the B-conventional surgical evacuation. Blood loss,surgical success rate, postoperative [3-HCG fall tim~, menstrual recovery and subsequent pregnancy outcome of the two groups were observed. Results Success rate of surgery of the hyst- eroscopic treatment group was 100% ,of the conventional treatment group was 80% ,and the difference was statistically significant（ P 〈0.05 ）. Intraoperative blood loss, postoperative recovery period time, β-HCG fall time of the hysteroscopic treatment group were lower than the conventional treatment group, and the differences between the two group had statistical significance（ P 〈 0.05 ）. Pregnancy of the hysteroscopic treatment group was 69.44% ,of the conventional treatment group was 40%, and the difference was statistically significant（P 〈 0.05 ）. Conclusion Hysteroscopy can further diagnose cornual pregnancy, and can find other intrauterine diseases at the mean time. Hysteroscopy treatment for cornual pregnancy is a safe and effective method with less blood loss and minimal invasion, which is better than the conventional surgical evacuation.
