Correlation between soluble intercellular adhesion molecule-1 and vascular endothelial function in elderly patients with isolated systolic hypertension

摘  要:目的探讨可溶性细胞间黏附分子1(sICAM-1)与老年单纯收缩期高血压(ISH)血管内皮功能的关系。方法采用横断面研究方法,连续收集2011-07-2012-07入住新疆维吾尔自治区人民医院干保二科的老年ISH患者110例作为ISH组,年龄65~95(74.2±7.3)岁;老年原发性高血压(EH)患者110例作为EH组,年龄65~94(72.3±7.3)岁;并选择年龄与之相匹配的正常血压人群80人作为对照组,年龄65~92(73.3±5.5)岁。采用ELISA法测定血清sICAM-1、晚期糖基化终末产物(AGE)浓度,Griess法测定血清一氧化氮含量;运用超声技术测定血流介导的内皮依赖性血管舒张功能(FMD),动脉硬化检测仪测定颈股动脉脉搏波传导速度(cfPWV)。采用直线相关分析及多元线性逐步回归分析法分析sICAM-1与其他指标的相关性。结果与对照组比较,老年ISH组、EH组sICAM-1[(191.52±37.59)、(137.28±27.70)比(88.84±20.31)μg/L]、AGE[(285.48±46.60)、(176.69±22.42)比(108.76±17.46)μg/L]、cfPWV[(1798.3±58.4)、(1548.8±105.2)比(975.4±101.6)cm/s]水平升高(均P<0.05),一氧化氮[(7.30±1.78)、(8.45±2.75)比(16.04±2.70)μmol/L]、FMD[(11.95±5.07)%、(12.25±3.34)%比(21.40±6.32)%]降低(均P<0.05)。相关分析得出,sICAM-1与AGE、cfPWV呈正相关(分别r=0.728,0.752,均P<0.05);sICAM-1与一氧化氮、FMD呈负相关(分别r=-0.794,-0.669,均P<0.05);多元线性逐步回归分析显示sICAM-1、一氧化氮是影响FMD的主要因素(分别β=-0.438,0.315,均P<0.05)。结论老年ISH患者血清sICAM-1水平明显升高,且与FMD呈负相关。Objective To investigate the relationship between soluble intercellular adhesion molecule-1 (sICAM 1) and vascular endothelial function of elderly patients with isolated systolic hypertension (ISH). Methods From Ju-ly, 2011 to July, 2012, 220 inpatients were enrolled, including 110 ISH patients age from 65 to 95 [mean age of ( 74.2±7.3) ] and 110 EH patients age from 65 to 94 [mean age of ( 72.3 ± 7.3 ) ]. Meanwhile 80 normotensive participants age from 65 to 92 [age of( 73.3 ± 5.5)] were served as control. Serum soluble intercellular adhesion molecule-1 (sICAM-1) and advanced glycation end products (AGE) concentration were measured by ELISA. Nitric oxide (NO) was determined by Griess method, the association between sICAM-1 and FMD and cfPWV were described by linear correlation analysis and multiple linear stepwise regression analysis. Results Compared with those of control group, slCAM-1 [(191.52 ± 37.59), (137.28±27.70) vs (88.8±9 20.31)μg/L], AGE [(285.48±46.60), (176.69±22.42) vs (108.76±17.46)μg/L], cfPWV [(1798.3±58.4), (1548.8±105.2) vs (975.4±101.6)cm/s] in ISH and EH groups were higher (all P〈0.05), while the concentration of NO [(7.30±9 1.78), (8.45±2.75) vs (16.0±2.70)μmol/L] and FMD [(11.95±5.07)%, (12.25±3.34)% vs (21.40±9 6.32)%] were lower (all P〈0.05). sICAM-1 positively correlated with AGE and efPWV (r=0. 728, P〈0.05; r=0.752, P〈0.05), but negatively correlated with NO and FMD (r=-0.794, P〈0.05; r=-0.669, P〈 0.05). Multiple linear regression analysis showed that sICAM-1 and NO were the important factors of FMD (β=-0. 438, β=0. 315, all P〈0. 05). Conclusion sICAM-1 significantly elevated in elderly patients with isolated systolic hypertension and negatively correlated with FMD.
