不同阶段慢性肾脏病患者的25（OH）D3水平及相关性分析

摘　　要：目的为了解本地区不同阶段慢性肾脏病（chronic kidney disease，CKD）患者血清中25（OH）D3的水平，并探讨其水平与其他检测指标的关系。方法选择2013年1月至3月在我院肾病科住院的CKD非透析患者119例及同时期在我院体检的健康对照者30名（健康对照组），采用酶联免疫法检测所有入选者25（0H）D3的水平，比较2组间25（0H）D3含量的差异，并分析其与体内其他生化指标的相关性。结果与健康对照组相比，CKD3～5期患者血清25（0H）D3含量均显著降低（PG0．05），CKD患者血清25（OH）D3缺乏率达37．8％，不足率高达60．5％；CKD3～5期患者的血清甲状旁腺激素（PTH）、血肌酐（SCr）水平均显著升高（PG0．05），CKD5期患者的血磷水平显著升高（P〈0．05）；25（OH）I）3与SCr、PTH、年龄、血糖（GLU）呈负相关（r=-0．480、-0．217、-0．604、-0．183，均PG0．05）；25（0H）D3与估算肾小球滤过率（eGFR）、血红蛋白（Hb）、血白蛋白（Alb）、性别、原发病种类呈正相关（r=0．569、0．512、0．359、0．200、0．300，均PG0．05）。结论本地区CKD3～5期患者的25（0H）D3水平明显下降，且CKD患者的25（OH）D3水平与SCr、eGFR、Alb、Hb、PTH等均存在一定的相关性。Objective To understand serum 25（OH）D3 level at different stages of chronic kid- ney disease （CKD） patients in this region and study the relationship between 25（OH）D3 levels and other detection indexes. Methods 119 cases of CKD at first stage to fifth stage not subject to dialysis admitted in our hospital from January 2013 to March 2013 and 30 cases of healthy controls examined in our hospital at the same period were selected. The serum levels of 25（OH）D3 were determined by u- sing enzyme-linked immunosorbent assay, compared between groups, and the relationship between the levels of 25 （OH）D3 and other detection indexes was analyzed. Results As compared with control group, the levels of 25 （OH）D3 in the CKD patients from the third stage to the fifth stage were signifi- cantly decreased （P^0. 05）, the insufficient rate of 25（OH）D3 in the CKD patients was 37. 8~ and the rate of deficiency was 60. 5 %. The levels of PTH and serum creatinine in the CKD patients from the third stage to the fifth stage were significantly increased （P〈0. 05）. The levels of serum phos- phate in the CKD patients at the fifth stage were significantly increased （P〈0. 05）. The levels of ser- um 25 （OH） D3 were negatively correlated with creatinine, PTH, age and blood glucose （r = - 0. 480, - 0. 217, - 0. 604 and - 0. 183 respectively, P〈0. 05 for all）, and positively with eGFR, hemoglo- bin, albumin, sex and types of the primary diseases （r= 0. 569, 0. 512, 0. 359, 0. 200 and 0. 300 re- spectively, P%0. 05 for all）.
