孕妇牙周病与早产低出生体重儿关系的Meta分析

摘　　要：目的：运用Meta分析循证孕妇牙周病与早产低出生体重儿（PLBWI）的关系，为PLBWI的预防控制提供参考依据。方法：搜集国内有关孕妇牙周病与PLBWI关系的前瞻性队列研究和病例对照研究，对数据进行整理和异质性检验后，运用RevMan5．1统计软件进行Meta分析。结果：共检出相关文献209篇，最终纳入9篇文献（5361例），其中病例对照研究5篇（4525例），合并OR=2．84，95％CI：2．07—3．89；队列研究4篇（836例），合并RR=6．38，95％CI：4．11～9．91。采用随机效应模型进行分析后，总效应测定结果显示牙周病孕妇组与正常健康孕妇组PLBWI出生率比较差异有统计学意义（RR=3．12，95％CI：2．02～4．82，Z=5．12，P〈0．00001）。结论：孕妇牙周病可能与早产低出生体重儿的发生具有相关性。Objective： To explore the relationship between maternal periodontal diseases and preterm low birth weight infants （ PL- BWI） based on Meta analysis, and provide a reference basis for prevention and control of PLBWI. Methods： Prospective cohort studies and case -control studies about the relationship between maternal periodontal diseases and PLBWI published in China were collected, then the data were summarized and analyzed by heterogeneity test, Meta analysis was performed on RevMan 5.1 statistical software. Results： A total of 209 related literatures were selected, and 9 literatures （5 361 cases） were included ultimately, including 5 case- control studies （4 525 cases） （combined OR = 2. 84, 95% CI： 2. 07 - 3.89） and 4 prospective cohort studies （ 836 cases） （ combined RR = 6. 38, 95% CI： 4. 11 -9. 91 ） . There was statistically significant difference in the birth rate of PLBWI between maternal periodontal diseases group and nor- mal healthy pregnant women group after random effect model analysis （ RR = 3.12, 95 % CI： 2. 02 - 4. 82, Z = 5.12, P 〈 0. 000 01 ） . Con- clusion： Maternal periodontal diseases might be correlated with the occurrence of PLBWI.
