解剖形锁定钛板内固定治疗MasonⅡ型和Ⅲ型桡骨头骨折

摘　　要：目的分析解剖形锁定钛板内固定治疗MasonⅡ、Ⅲ型桡骨头骨折的疗效。方法对自2008—01—2011—12收治的28例MaSOnⅡ、Ⅲ型桡骨头骨折，采用切开复位解剖形锁定钛板内固定术进行治疗，术中有4例行同种异体骨植骨，8例行内侧副韧带修复。结果28例均获得随访，按照Broberg和Money的肘关节功能评分标准进行评分，评分为平均86分（46．100分）；肘关节屈曲平均118°（95～135°），伸直平均20°（0-30°），旋前平均52°（15～60°），旋后平均58°（20—70°）；本组：优15例，良10例，中3例，优良率89．3％。所有患者肘关节功能恢复良好，仅1例遗留肘部轻度疼痛，2例肘、腕部轻度无力。结论采用切开复位解剖锁定钛板内固定术治疗MasonⅡ、Ⅲ型桡骨头骨折临床疗效满意，内固定牢固，可早期对I能宅目争楼．Objective To assess clinical outcomes of anatomical locking compression titanium plate for treatment of patients with Mason type II or III radial head fractures. Methods A total of 28 patients from Jan. 2008 to Dec. 2011 with Mason type lI or III radial head fractures were recruited in this clinical retrospective analysis. All of the patients were treated by open reduction and anatomical locking compression titanium plate fixation. Allograft bone grafts were performed in 4 of 28 cases and medial collateral ligaments were reconstructed in 8 cases. Results All of 28 patients were followed up. According to Broberg and Money Elbow Performance Index, the average score was 86 points（range, 46-100 points）. The average flexion mobility was l18°（range,95-135°）, extension 20°（range, 0-30°）, pronation 52°（range, 15-60°）, supination 58°（20-70°）. The results were excellent in 15 eases, good in 10 cases, fair in 3 cases, with an excellent and good rate of 89.3%. All patients recovered with good function of the elbows. One patient had moderate pain at the elbow, and 2 patients had some loss of strength of the elbow and wrist. Conclusion Open reduction and anatomical locking compression titanium plate internal fixation are option for Mason type II or 11I radial head fractures, which provide finn internal fixation and the feasibility of early functional excereise.
