摘  要:目的探讨七叶皂苷钠对创伤性急性肺损伤的治疗作用。方法选择80例创伤性急性肺损伤患者,随机分为七叶皂苷钠治疗组(A组,n=40)和对照组(B组,n=40),对照组接受常规治疗。七叶皂苷钠治疗组在常规治疗基础上静脉滴注七叶皂苷钠20mg,1次/d,连用12d,观察两组的临床疗效并检测治疗前及治疗后第3、6、9、12d的血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)和白细胞介素-8(IL-8)水平变化。结果七叶皂苷钠治疗组与对照组在治疗前TNF-α、IL-1、IL-6和IL-8水平无差异。治疗6d后,七叶皂苷钠治疗组的TNF-α、IL-1、IL-6和IL-8水平下降较对照组明显(P<0.05)。七叶皂苷钠治疗组治疗后呼吸频率(RR)、氧分压(PaO2)、二氧化碳分压(PaCO2),氧合指数(PaO2/FiO2)与对照组比较差异有统计学意义(P<0.05)。结论七叶皂苷钠可显著抑制机体炎症递质的产生,清除急性肺水肿,在创伤性急性肺损伤早期应用治疗效果明显。Objective To observe the therapeutic value of aescine in traumatic acute lung injury. Meth- ods Eighty patients with traumatic acute lung injury were randomly divided into aescine group ( n = 40 ) and con- trol group ( n = 40 ). Both groups was given routine treatment, while the patients of the aescine group were given aescine (20rag,intravenous drip, once per 24 hours, for 12 days). The efficacy in both groups was assessed, and their serum cytokines including tumor necrosis factor-α(TNF-α) ,interleukin-1 ( IL-1 ) ,interleukin-6 (IL-6) and in- terleukin-8 ( IL-8 ) were measured on the day before treatment and on the third day, the sixth day, the ninth day and the twelfth day after treatment. Results There was no difference in TNF-α, IL-1, IL-6 and IL-8 levels between the two groups before treatment, which reduced more significantly on the sixth day after treatment in the aescine group than those in the routine group ( P 〈 0. 05 ). There were significantly differences in the respiratory rate ( RR ) , arteri- al partial pressure of oxygen ( PaO2 ) , arterial partial pressure of carbon dioxide( PaCO2 ) and PaO2/FiO2 after treat- ment between the two groups ( P 〈 0. 05 ). Conclusion Aescine significantly inhibits the production of inflammato- ry eytokines, relieves acute pulmonary edema. Early administration of aescine can play an important role in the treatment of acute lung injury.
