全结肠系膜切除在乙状结肠癌根治术中的应用

摘　　要：目的探讨乙状结肠癌根治术中应用全结肠系膜切除的优势。方法回顾性分析2011年1月至2012年12月在我院接受全结肠系膜切除(42例)和传统结肠癌根治术(46例)的乙状结肠癌患者的临床资料。研究全结肠系膜切除在淋巴结清扫、减少术中出血以及输尿管和自主神经的保护等方面的优势。结果全结肠系膜切除组的术中出血、术后排尿困难的发生率明显少于传统手术组;在淋巴结清扫、输尿管保护方面有一定优势,但未呈现出统计学差异。结论全结肠系膜切除手术在减少术中出血、自主神经保护方面较传统的乙状结肠癌根治术有明显优势,且不增加手术并发症。Objective To investigate the superiority of complete mesocolic excision （CME） in radical sigmoidectomy. Methods Clinical data of the patients accepted CME （42 cases） and patients accepted traditional radical colectomy （46 cases） with sigmoid colon cancer from January 2011 to December 2012 in our hospital was retrospectively dissection, reducing blood loss, and protection of analyzed. The advantage of CME in lymph node ureter and autonomic nerve were studied. Results Blood loss, postoperative dysuria in CME group were significantly less than traditional surgery group. In the lymph node dissection and ureteral protection, CME group had certain advantages, but without statistically significant difference. Conclusion Compared to traditional radical sigmoideetomy, CME has obvious advantages in reducing blood loss, autonomic nervous protection and without increasing complications.
