摘  要:目的研究β-七叶皂甙钠对大鼠脑梗死后脑水肿的治疗作用。方法12只雄性SD大鼠随机分为对照组(n=6)及治疗组(n=6),采用线栓法阻塞大鼠大脑中动脉,制作局灶性脑梗死模型,治疗组在造模后即刻向腹腔内注射β-七叶皂甙钠(5mg/kg(i),对照组注射等量生理盐水,在造模后6h、1、3、5、7d行磁共振成像(MRI)扫描,测量计算脑梗死体积、水肿吸收率、梗死区T1加权像(T1WI)、T2加权像(T2WI)、液体衰减反转恢复序列(FLAIR)、弥散加权成像(DWI)序列的信号强度比(SIR)及信号强度比相对变化率(ASIR)。结果治疗组在造模后3、5、7天时的脑梗死体积均明显小于对照组(P〈0.05-0.01);脑梗死后7天相对于3天的水肿吸收率明显高于对照组(P〈0.05);在造模后3、5、7天治疗组T1WI的SIR均明显高于对照组,T2WI及FLAIR序列的的SIR均明显低于对照组(P〈0.05~0.01),在7天时DWI的SIR明显低于对照组(P〈0.01);治疗组T2WI、FLAIR及DWI的ASIR均明显高于对照组(P〈0.05~0.01)。结论β-七叶皂甙钠对大鼠梗死后脑水肿有明显的治疗作用。Objective To study the therapeutic effect of β-aescinum natrium on brain edema after cerebral infarction in rats. Methods Twelve male SD rats were randomized into control group ( n=6 ) and treatment group ( n=6 ) . The model of focal cerebral infarction was established with obstructing middle cerebral artery of rat by suture emboli. Rats of treatment group were treated by injecting β-aescinnm natrium ( 5 mg/kg ~ d ) into abdominal cavity immediately after the models were established. The same amount of normal saline was injected into the control group rats. Magnetic resonance imaging (MRI) was performed at 6 h, 1, 3, 5 and 7 d after the success of model making. Cerebral infarction volume, edema absorption rate, signal intensity ratio ( SIR ) of T1 weighted imaging( T1WI),T2 weighted imaging(T2WI), fluid-attenuated inversion recovery (FLAIR) and diffusion weighted imaging (DWI) sequences in the infarction zone and relative variance ratio of SIR ( ASIR ) were measured and calculated. Results In the treatment group, the cerebral infarction volumes at 3, 5, 7 d after the models were significantly less than in the control group (P 〈 0.05 - 0.01 ) ,the edema absorption rate, which was the 7th day against the third day after infarction, was significantly higher than in the control group ( P 〈 0.05 ) .At 3, 5, 7 d, the SIR of T1WI were all higher than in the control group, and the SIR of T2WI and FLAIR were all lower than in the control group (P 〈 0.05 - 0.0l ) . The SIR of DWI in the treatment group was obviously lower than in the control group at 7 d (P 〈 0.05 ) .The ASIR of T2WI, FLAIR and DWI were significantly higher than in the control group (P 〈 0.05 - 0.01 ) . Conclusions β-aescinum natrium has a significant therapeutic effect on brain edema after cerebral infarction.
