两种径路冠心病介入诊疗术后患者外周血管并发症的原因分析及护理

摘　　要：目的:探讨两种径路冠心病介入诊疗术后外周血管并发症的原因、处理及护理。方法:对我院2008年1月～2012年12月2327例行冠心病介入诊疗的患者,其中经股动脉径路984例,经桡动脉径路1343例,术后出现外周血管并发症的情况进行回顾性分析。结果:984例经股动脉径路的患者术后局部血肿42例,假性动脉瘤9例,动静脉瘘7例,腹膜后血肿1例,动脉血栓1例,下肢静脉血栓1例;在1343例经桡动脉径路的患者中术后局部血肿27例,前臂血肿20例,桡动脉痉挛11例,桡动脉闭塞8例,腕管综合症1例,骨筋膜腔综合症1例。结论:两种径路冠心病介入诊疗在要求介入医生的操作规范及技巧的基础上,术后应加强病房护士的管理,提高专业技能,注意观察细节变化,预见潜在的危险因素,有针对性、有目的进行观察及护理,可减少术后并发症。Objective： To investigate the treatment of two kinds of path of coronary heart disease diagnosis and treatment of peripheral vascular complications, the reasons and nursing. Methods： the patients of our hospital in 2008 January 2012 December 2327 cases of coronary intervention, including by femoral artery approach in 984 cases, the radial artery in 1343 cases, a retrospective analysis of the presence of peripheral vascular complications after surgery. Results： in the 984 cases via femoral artery approach for patients with local hematoma in 42 cases, 9 cases of false aneurysm, arteriovenous fistula in 7 cases, 1 cases of retroperitoneal hematoma, 1 cases of arterial thrombosis, 1 cases of venous thrombosis of the lower limbs in 1343 cases; through radial artery pathway in patients with postoperative local hematoma in 27 cases, 20 cases of forearm hematoma, 11 cases of radial artery spasm, radial artery occlusion in 8 cases, 1 cases of carpal tunnel syndrome, 1 cases of bone compartment syndrome. Conclusion： two kinds of operation norm approach coronary artery interventional cardiologists in the requirements and skill, strengthening ward nurses should postoperative management, improve the professional skills, to observe the details of the changes, foreseeing the potential risk factors, targeted, with the purpose of observation and nursing, and can reduce postoperative comolications.
