超声止血刀在81例妇科腹腔镜手术中的应用

摘　　要：目的　探讨在妇科腹腔镜手术中使用超声刀止血的效果。方法　选取应用超声刀进行腹腔镜妇科手术共 81例 ,术式包括经阴道腹腔镜辅助全子宫切除术 (LAVH)、腹腔镜下子宫肌瘤剜除术、腹腔镜下附件切除术 ,并与同期应用电凝术或Endo -GIA +电凝术止血的相同术式进行比较。结果　使用超声刀较电凝术及Endo-GIA术中出血量明显减少 (P <0 0 5 ) ,手术时间显著缩短 (P <0 0 5 ) ,无术中及术后并发症。结论　腹腔镜下应用超声刀止血具有缩短手术时间 。Objective To evaluate the effects of harmonic scalpel on homeostasis in laparoscopic surgery.Methods We compared 81 cases undergoing laparoscopy assisted vaginal hysterectomy,laparoscopic myomectomy or laparoscopic salpingo-oophorectomy using harmonic scalpel with the ones using monocoagulation or Endo-GIA plus monocoagulation simultaneously.Results Compared with monocoagulation and Endo-GIA plus monocoagulation,the use of harmonic scalpel in laproscopic surgery could significantly shorten the operation time(P<0.05) and lessen the blood loss(P<0.05).No operative and post-operative complications were observed.Conclusion The use of harmonic scalpel in gynecologic laparoscopy can shorten operation time and achieve a better result of homeostasis.
