视黄醇结合蛋白-4与初诊2型糖尿病患者血脂及胱抑素C的关系

摘　　要：目的:探讨视黄醇结合蛋白-4(RBP-4)与初诊2型糖尿病(T2DM)患者血脂及胱抑素C(Cysc)的关系。方法:选取T2DM(T2DM组)患者85例,体检健康者(对照组)65例,测定两组空腹血糖(FPG)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、RBP-4及游离脂肪酸(FFA),计算体重指数(BMI)及腰臀比(WHR),并作相关性分析,同时测定不同阶段糖尿病肾病患者的RBP-4及Cysc水平。结果:与对照组比较,T2DM组BMI、FPG、TG、LDL-C、RBP-4及FFA明显升高,但HDL-C明显降低,差异均有统计学意义(P<0.05)。RBP-4与FPG、BMI、TG及总胆固醇(TC)均呈正相关,FFA是影响血清RBP-4水平的明显因素。T2DM组患者Cysc及RBP-4水平均明显高于正常对照组,差异有统计学意义(P<0.05)。随着T2DM尿蛋白含量的增加,其Cysc及RBP-4水平明显上升。结论:T2DM患者RBP-4水平与血脂水平正相关,而RBP与Cysc同时检测可作为评价肾功能受损程度及部位的指标。Objective.- To evaluate the relationship of retinol binding protein 4（RBP-4） with hyper- lipidemia and cystatin C（Cysc） in the patients with newly-diagnosed type 2 diabetes. Methods： Serum sam- ple were taken from 85 patients with T2DM, and 65 normal controls. FPG, TG, HDL-C, LDL-C, RBP- 4, FFA, BMI and WHR were measured. Spearman analysis and Logistic analysis were used. RBP-4 and Cysc at different stage of diabetic nephropathy were measured. Results; Compared the control group, BMI, FPG, TG, LDL-C, RBP-4 and FFA of the T2DM group were significantly increased, but the HDL- C was significantly reduced（P〈0.05）. RBP-4 was positively correlated FPG, BMI, TG and TC.FFA was the strongest influencing factor of RBP-4 concentration. The level of the Cysc and RBP-4 of the T2DM group were significantly higher than those of the control group（P〈0.05）. With the increase of uri- nary protein of T2DM , the Cysc and RBP-4 level rise obviously. Oonclusions： RBP-4 of the T2DM was positively correlated with hyperlipidemia, and RBP and Cysc could be taken as the indicators for evaluating the extent and location of diabetic nephropathy.
