大腿肌萎缩对半月板成形或缝合后疗效的影响

摘　　要：目的探讨半月板成形及缝合术后股四头肌功能锻炼的重要性。方法选取2008年10月～2011年4月于该科行关节镜下半月板成形或缝合术，术后3个月时Lysholm评分低于80分的患者12例，指导其积极功能锻炼，术后6个月时测量髌上10及20cm水平大腿周径、Lysholm评分、VAS评分、与术前及术后3个月时进行比较。结果术后3个月髌上10及20cm水平大腿周径小于术前，两者比较差异有显著性（P〈0．05），VAS疼痛评分较术前显著升高（P〈0．05），Lysholm评分可10例，差2例。术后6个月较术后3个月髌上10及20cm水平大腿周径显著增加（P〈0．05），Lysholm评分显著升高（P〈0．05），VAS疼痛评分显著下降（P〈0．05）。术后6个月较术后3个月髌上10及20cm水平大腿周径增加与Lysholm评分增加呈显著正相关（P〈0．05），与VAS疼痛评分减少呈显著正相关（P〈0．05）。结论半月板损伤患者行半月板成形或缝合后容易出现大腿肌萎缩，积极的股四头肌功能锻炼有利于患者的康复。[ Objective ] To approach the importance of functional exercise of quadriceps muscle to the knee function after meniscal plasty and repair. [ Methods ] From October 2008 to April 2011, 12 patients were included in this study, who received meniscal plasty and repair under arthroscopy but the Lysholm scores at 3 months postop- eratively were lower than 80. After 3 months of positive functional exercise, thigh circumference at 10 cm and 20 cm proximal to patella, Lysholm scores and VAS scores were compared with preoperation and 3 months postoperatively. [Results ] At 3 months postoperative thigh circumference at 10 em and 20 em proximal to patella were significantly less than preoperation （P 〈0.05）, VAS scores were significantly higher than preoperation （P〈0.05）, the Lysholm scores showed 10 fair and 2 poor. At 6 months postoperative, thigh circumference at 10 em and 20 cm proximal to patella and the Lysholm scores were significantly higher compared with 3 months postoperative （P〈0.05）, the VAS scores were decreased significantly （P 〈0.05）. The increase of thigh circumference at 10 cm and 20 cm proximal to patella from 3 month to 6 month postoperative were significantly correlated with the increase of Lysholm scores and decrease of VAS scores. [ Conclusion ] The patients with meniseal lesion were susceptible to atrophy of thigh muscle after meniscal plasty and repair, positive functional exercise of quadriceps muscle was favorable to rehabilitation postoperative.
