输尿管镜气压弹道碎石术436例疗效分析

摘　　要：目的：评价经输尿管镜气压弹道碎石取石术治疗输尿管中下段结石的临床疗效。方法：回顾分析自2008年3月~2012年12月应用输尿管镜气压弹道碎石取石术治疗的输尿管中下段结石患者436例的临床资料。结果：手术时间12~53 min。425例输尿管镜下气压弹道碎石取石成功,占93.8%（425/453）,随访1~3个月,结石排净率为98.1%（417/425）,19例患者因输尿管下段狭窄、扭曲、息肉、畸形等原因无法进入输尿管镜。术后出现发热、腰痛、血尿并发症35例次（35/453）。术后长期随访未见明显并发症。结论：经输尿管镜气压弹道碎石术治疗输尿管中下段结石安全、有效、创伤小。Objective To evaluate the clinical efficiency of pnenumatic lithotripsy under ureteroscope for the treatment of middle and lower ureteral calculi.Method 436 data of middle and lower ureteral calculi patients treated by ureteroscopic pnenumatic lithotripsy during March 2008 to December 2012 were analyzed retrospectively.Results The operative time was 12 to 53 minutes.425 cases underwent successfully and the successful rate was 93.8%（425 /453）.The stone-clearance rate was 98.1%（417 /425） after 1 to 3 months follow up.19 cases were failed because of ureteral stenosis,twist,polypus or misshapen.Fever,lumber pain or hemuresis after operation was observed in 35 patients.There was no sereve complications during long-time follow up.Conclusion Pnenumatic lithotripsy under ureteroscope is safe,effective,minimally invasive for treatment of ureteral calculi in inferior segment and middle segment.
