糖尿病肾病患者对疾病认知的现状及其相关因素分析

摘　　要：目的了解糖尿病肾病(diabetic nephropathy,DN)患者对疾病的认知现状,并对影响认知的相关因素进行分析。方法采用自制问卷对2010年8月至2011年12月在上海交通大学附属第六人民医院内分泌代谢科住院的98例DN患者和100例糖尿病(diabetes mellitus,DM)患者进行调查。结果DN患者大专及以上学历比例为15.3%,低于DM组的46.0%(P<0.01)。DN组和DM组的认知评分分别为(32.91±23.57)分和(45.55±23.43)分,差异有统计学意义(P<0.01);且不同文化程度DN患者的认知评分存在差异(P<0.01)。以DN为因变量,对各种因素进行回归分析,仅年龄进入回归方程(Waldχ2为6.327,P=0.012),认知评分Waldχ2为3.391(P=0.066)。结论 DN的发生、发展与患者对疾病相关知识的掌握程度密切相关,因此护理人员在重视糖尿病健康宣教的同时,应针对性地对DN及DM患者开展并发症相关知识的教育。Objective To investigate the cognitive status and its influencing factors for patients with diabetic nephropathy.Methods A total of 198 inpatients from August 2010 to December 2011 were recruited,including 98 patients with diabetic nephropathy and 100 patients without.All of them were asked to fill in the self-made questionnaire of the knowledge about diabetic nephropathy.Results The proportion of patients with junior college education level in diabetic nephropathy group was 15.3％,which was lower than non-diabetic nephropathy group （46.0％） （P＜0.01）.Cognitive score in diabetic nephropathy group （32.91 ± 23.57） was also significantly lower than non-diabetic nephropathy group （45.55 ± 23.43）.Cognitive scores of different education degree of patients with diabetic nephropathy are different （P＜0.01）.Logistic regression analysis revealed that age was the only independent related factor to diabetic nephropathy（Wald x2 value 6.327,P=0.012）.Cognitive score was almost into the regression equation（Wald x2 value 3.391,P=0.066）.Conclusion The knowledge of diabetic nephropathy was closely related to its occurrence and development.Therefore,on the basis of the diabetes health education,we also should strengthen the knowledge about diabetic complications for diabetic patients.
