基于现代文献的灸法治疗荨麻疹的临床证据研究

摘　　要：目的:分析灸法临床实践高度相关的高质量证据,利用循证医学的方法,评价灸法治疗荨麻疹的临床研究,为治临床医生提供临床证据。方法:以灸法临床实践中常见的重要问题为出发点,全面检索当前可得的临床研究文献,根据循证医学五级分类标准,对于能够回答对应临床问题的证据,从高到低进行分类,采用RevMan5.0.20统计结局指标。结果:有13篇符合本研究需要。缺少高质量A级证据;B级证据表明灸法可能优于氯雷他定、盐酸西替利嗪片、咪唑斯汀片或与之相当,但不良反应少;C级证据表明灸法可能优于盐酸西替利嗪片或与盐酸西替利嗪片相当,但不良反应少。结论:灸法治疗荨麻疹的有效性是肯定的;灸法和西药相比很可能等效,今后可设计严格的等效性或非劣性试验来验证;灸法与西药并用的效果也有一定优势,尚需要高质量试验的证实。Objective： To analyze highly relevant high quality evidence in moxibustion clinical practice, through using the method of evidence-based medicine, and to evaluate moxibustion in the treatment of urticaria, providing clinical evidence for clinical doctors. Methods： Centered on moxibustion clinical practices, we retrieved the current available clinical research literature comprehensively, according to the evidence-based medicine category five classification standards, and searched for corresponding clinical evidence, from high to low classification. We used the RevMan5.0.20 for statistical outcome indicators. Results： There are 13 studies in this research. Class A evidences were lack. Class B evidenced that moxibustion may better than loratadine, cetirizine hydrochloride, imidazole sting or pretty, but less adverse reactions; Class C evidenced that moxibustion may better than cetirizine hydrochloride or pretty but less adverse reaction. Conclusion： Moxibustion treatment effectiveness of urticaria is certain; Moxibustion compared to western medicine may be equivalent, the future can be designed strictly non-inferiority or equivalence test to verify; Moxibustion and western medicine combined using has certain advantages, it needs high quality test confirmed.
