摘 要:目的:探讨早发型重度子痫前期和晚发型重度子痫前期发病机制的差异。方法取初诊初治的早发型、晚发型重度子痫前期患者和同期正常妊娠者分娩前清晨空腹静脉血4~5mL,用ELISA法测定各组样本血清中干扰素( IFN)-γ、白介素(IL)-10、IFN-γ/IL-10比值及可溶性血管内皮生长因子受体l(sFlt-1)的含量,并对结果进行分析。结果重度子痫前期患者血清中IFN-γ、IFN-γ/IL-10比值及sFlt-1含量均高于对照组,IL-10的含量低于对照组。早发型重度子痫前期血清中IFN-γ/IL-10比值变化与sFlt-1的变化具有相关性( r=0.941,P<0.001);而晚发型重度子痫前期患者的这种相关性无统计学意义。结论患者血清中IFN-γ、sFlt-1以及IFN-γ/IL-10比值升高致胎盘缺血缺氧是重度子痫前期致病因素之一,早发型与晚发型重度子痫前期在发病机制上存在差异。Objective To study the difference in the pathogenesis of early-onset severe preeclampsia (EOSP) and late-onset severe preeclampsia (LOSP).Methods Vein fasting blood 4-5mL were collected from newly diagnosed untreated patients with EOSP , LOSP and the normal control patients before delivery .Enzyme linked immunosorbent assay (ELISA) was used to detect interferon-γ(IFN-γ)level, interleukin-10 (IL-10) level, ratio of IFN-γ/IL-10 and soluble fms-like tyrosine kinase-1(sFlt-1) in the serum.Results Compared with the control group , serum level of IFN-γ, IFN-γ/IL-10 ratio and sFlt-1 level were higher but IL-10 level was lower in patients with severe preeclampsia.There was a positive correlation between IFN-γ/IL-10 ratio and sFlt-1 level in patients with EOSP (r=0.941,P〈0.001), but the correlation in patients with LOSP was not significant .Conclusion The excess of serum IFN-γ, sFlt-1 and elevated IFN-γ/IL-10 ratio lead to severe preeclampsia .The pathogenesis of EOSP and LOSP is different .
