输尿管镜微创碎石术与ESWL治疗输尿管结石的临床分析

摘　　要：目的比较输尿管镜微创气压弹道碎石术（URSL）与体外震波碎石术（ESWL）治疗输尿管结石的临床疗效。方法对1999年1月至2012年12月分别使用URSL与ESWL治疗输尿管结石741例患者的临床资料进行回顾分析。其中，输尿管镜治疗315例，用ESw．L治疗输尿管结石436例。结果两组患者术后一个月的结石排净率分别是URSL治疗总有效率为92．5％，明显高于ESWL治疗的总有效率79．1％（P〈0.05）；输尿管镜组的主要并发症为输尿管损伤、穿孔为3．5％，而ESWL组的主要并发症为再次治疗率高为10．9％。结论采用URSL与ESWL治疗输尿管中、下段结石疗效均确切、安全性高、创伤小、恢复快；而治疗输尿管上段结石用ESWL疗效较URSL理想。Objective To evaluate the clinical effect of ureteroscopic micropneumatic lithotripsy（URSL） compared with estracorporeal shock-wave lithotripsy （ ESWL ） for treatment ofureteric calculi. Methods During January 1999 and December 2008,741 patients with ureteral calculi were divided into two groups randomly and treated respectively with URSL or ESWL were analysed retrospectively, treatment with ESWL included 436 patients with ureteric calculi. URSL was used in 315 patients with ureteric calculi .Results The total efficacy rate of the URSL group and ESWL group resulted inastone-free rate were 92.5% and 79.1% （P〈0.05） , URSL group caused damnify, perforation of ureters in3.5% ,and ESWL group had a retreatment rate of 10.9% .Conclusion URSL and ESWL treatment of middle-lower uretaral calculi has been effective,safe,less invasive. ESWL is suitable for the treatment of choice for patients with upper ureteral calculi.
