宫腔镜与腹腔镜联合手术治疗妇科疾病的临床价值

摘　　要：目的探讨宫腔镜与腹腔镜联合治疗妇科疾病的安全性、可行性及有效性。方法回顾性分析2010年5月至2011年5月在我院应用宫腔镜与腹腔镜联合诊治53例妇科疾病患者临床资料。结果53例患者共施行118项操作，全部在宫腔镜与腹腔镜下完成，手术均顺利完成，无并发症发生；手术时间为30—120min，术后无需镇痛，6h后拔除导尿管，且术后24h内恢复肛门排气，出院后门诊随访3—12个月，17例不孕症患者术后妊娠8例，妊娠率为47．1％；纵膈子宫8例患者中足月妊娠4例、自然流产l例。结论宫腔镜与腹腔镜联合手术解决了单纯宫腔镜或腹腔镜治疗妇科疾病的局限性，可同时诊治盆腔内妇科疾病，在腹腔镜监护下行官腔镜操作，可提高对妇科疾病诊断的准确性及治疗的安全性与手术的有效性。Objective To discuss feasibility and effectiveness of the combined hysteroscopy and laparoseopy in treating gynecological diseases.Methods Were retrospectively analyzed in May 2010 to May 2011 in our hospital application of diagnosis and treatment of hysteroscopy and laparoscopy in combination with 53 patient clinical material.Asaresult A total of 53 patients of 118 operations, All done under hysteroscopy and laparoscopy, surgery were finished smoothly, No complications occurred during the operation; Operation time was 30 - 120rain, without the need for postoperative analgesia, 6h after removal of the catheter, anal exhaust recovery within 24 h and postoperative follow-up after discharge, 3 - 12 months, 17 cases of infertility patients after pregnancy in 8 eases, the pregnancy rate was 47.1%; Mediastinum uterus of 8 eases 4 eases of full- term pregnancy, spontaneous abortion in 1 case.Conclusion Hysteroseopy and laparoseopy operation solves the limitation of hysteroscopy or laparoscopy in treating gynecological diseases, At the same time make a diagnosis and give treatment of pelvic disease of department of gynaecology, In laparoscopic monitoring downward hysteroscopy operation, to improve the accuracy of the gynecological disease diagnosis and treatment of the effectiveness of the security and surgeryo
