摘　　要：目的探讨早期帕金森病（PD）患者抑郁状况与健康相关生活质量（health—relatedqualityoflife，HR-QOL）的关系。方法筛选出87例早期PD患者入组。采用统一帕金森病评分表（UPDRS）和Hoehn-Yahr评价运动症状，采用流行病学研究中心编制的抑郁量表（CES．D）对抑郁症状进行评价，采用36条目简化医疗结局调查问卷（SF·36）评价HR-QOL。探讨早期帕金森病（PD）患者抑郁状况与健康相关生活质量（health．relatedqualityoflife，HR—QOL）的关系。结果87例早期PD患者中有51例存在抑郁状态（CES．D≥16分），无抑郁状态患者36例（对照组）。抑郁状态组患者除躯体疼痛（BP）维度外，其余各维度分值较对照组均下降。躯体功能（PF）；躯体问题导致的角色受限（RP）；总体健康（GH）；活力（vT）；社交功能（sF）；情感问题导致的角色受限（RE）；精神健康（MH）等维度评分均明显低于对照组（Ps0．05）。结论抑郁症状严重影响早期PD患者的HR．QOL，是导致早期PD患者HR-QOL恶化的主要因素。Objective To investigate the relationship between depression and health-related quality of life in patients with early Parkinson＇s disease. Methods All 87 patients with early PD were identified in a clinical-based study. Motor functions were measured by Unified Parkinson＇s Disease Rating Scale （UPDRS） and Hoehn-Yahr Scale. Depressive symptoms were assessed by Center of Epidemiological Survey Depression Scale （CES-D）. HR-QOL was measured by SF-36. To study the relationship between depression and health-related quality of life in patients with early Parkinson＇s disease. Results In all 87 patients with early PD ,51 patients were in the depression state （ CES-D〉16 score ） , and 36 patients were not in the depression state（control group）. Except bodily pain dimension, patients with early PD in the depression state had a lower score on all dimensions of SF-36, including physical function, role limitation-physical, general health, vitality, social function, role limitation-emotional and metal health. Compared with control group, the score decreased respectively with significance （P 〈 0.05 ） .Conclusions Depressive symptoms has a substantial impact on HR-QOL in patients with early Parkinson＇s disease. Depressive symptoms correlated strongly with lower quality of life.
