误诊为盘状红斑狼疮的皮肤结核病1例

摘　　要：患者女,54岁。双侧面颊、鼻梁渐发蝶形分布的浸润性红斑、萎缩性瘢痕20年,加重4个月余,曾多次被误诊为盘状红斑狼疮。实验室检查:血沉:36mm/h;结核抗体:弱阳性。PPD试验:(2+)。皮损组织病理示:表皮突消失,真皮内见一结核结节,其中心可见郎汉巨细胞,未见明显的干酪样坏死,外周绕以淋巴细胞浸润。二联抗结核治疗有效。A 54-year-old female presented with infiltrated erythema, atrophic sear over the double cheeks and nose for more than 20 years, aggravated for more than 4 months which was misdiagnosed as discoid lupus erythemato- sus for twenty years. Abnormal laboratory tests were as follows ： erythrocyte sedimentation rate （ ESR ） was 36mm/h, anti-tuberculosis antibody was weakly positive, purified protein derivative skin test （PPD）was strongly positive. Histopathologic examination showed that trochanterellus disappeared, a tubercular nodule was in the dermis, in the center of which were Langerhans giant cells, caseous necrosis was not found, and around which were lymphocytes. Treatment of duplex anti-tuberculosis was effective.
