摘 要:目的探讨常规彩超联合超声弹性成像诊断弥漫性甲状腺肿(通称原发性甲亢或Graves病)和桥本氏甲状腺炎(HT)的价值。方法选择103例Graves病和103例HT患者。常规彩超观察甲状腺大小、形态、回声特征、血流及周围组织情况,通过超声弹性成像获取弹性分级、相对应变值平均值和蓝色面积比例等信息。比较常规彩超与联合超声诊断准确率。结果 Graves组和HT组的甲状腺左、右叶前后径、峡部厚度、回声分级、血流分级差异不具有统计学意义(P>0.05)。Graves组甲状腺回声以散在低回声分布为主,HT组以弥漫性低回声分布为主。Graves组左、右侧上动脉内径、PSV、EDV大于HT组。Graves组弹性评分多为2级,占75.73%;HT组弹性评分多为3级,占73.79%。Graves组MEAN明显大于HT组,%AREA反之,两组比较差异具有统计学意义(P<0.05)。常规彩超诊断Graves病和HT的准确率为63.11%、72.82%,联合超声准确率为81.55%、93.20%。结论在诊断Graves病和HT中,常规彩超联合超声弹性成像能够获得更多的信息,达到较高的诊断准确率。Objective To approach the value of conventional color doppler ultrasound combined with ultrasound elastography in diagnosing diffuse goiter (primary hyperthyroidism or Graves" disease) and Hashimoto's thyroiditis (HT) value. Methods 103 patients who were diagnosed as Graves" disease and 103 patients who were diagnosed as HT were chosed. Thyroid size, shape, echo characteristics, blood and surrounding tissue conditions were observed with using of color doppler ultrasound. Secondly, Elasticity imaging classification, the average value of the relative strain and the blue area ratio and other information were gained by ultrasound elastography. The diagnostic accuracy of color doppler ultrasound and ultrasound elastography were compared. Results The anteroposterior diameter of thyroid left lobe and right lobe, thickness of isthmus, thyroid echo levels and blood flow classification in Graves group and HT group have not significant difference (P^0.05). The thyroid echo in Graves group was scattered hypoechoic echo- based distribution. The thyroid echo in Hashimoto's thyroiditis group mainly was diffuse hypoechoic echo-based distri- bution. The left and right artery diameter, PSV, EDV in Graves group were greater than Hashimoto's thyroiditis group. The elastic score in Graves group mainly were grade 2, accounting for 75.73%; the elastic score in Hashimoto 's thyroiditis group mainly were grade 3, accounting for 73.79%. MEAN in Graves group was significantly greater than Hashimoto's thyroiditis group, ~ AREA is on the contrary, and the difference was statistically significant (P% 0.05). The accuracy of color doppler ultrasound in diagnosing Graves' disease or Hashimoto's thyroiditis respectively was 63.11%, 72.82 %, while the accuracy of combined ultrasound respectively was 81.55 %, 93.20 %.
