周围性肺结核瘤超声造影表现

摘　　要：目的总结周围性肺结核瘤超声造影表现，探讨超声造影对周围性肺结核瘤的诊断价值。方法对21例病理证实的周围性肺结核瘤的超声造影资料进行分析，分析增强模式及始增时间，达峰时间，峰值强度。结果2l例肺结核瘤，超声造影12例（57．1％）表现为环形增强，5例（23．8％）为均匀增强，4例（19．1％）为不均匀增强。其始增时间、达峰时间及峰值强度呈偏态分布，其中位数及第25％和第75％值范围分别为14S，9～14S、22S，21～26S、83dB，55～92dB。结论超声造影环状增强模式有助于肺结核瘤的诊断；时间一强度曲线相关参数对肺结核瘤的诊断价值不大。Objective To describe the characteristics of pleural-based pulmonary tuberculoma by contrast en- hanced sonography and investigate their diagnostic performance. Methods 21 patients with pathologically proven pleural-based pulmonary tuberculoma underwent contrast-enhanced sonography examination. Based on visual observa-lion,enhancement patterns were analysed. Time to enhancement, time to peak enhancement and peak enhancement in- tensity derived from time intensity curve were also evaluated. Results （）f the 21 pleural-based pulmonary tuberculo- mas, 12（57. 1%） showed rim enhancement pattern, 5 （23.8%） showed homogeneous enhancement patterns, and 4 （19. 1%） showed heterogeneous enhancement patterns. The median and 25th to 75th interquartile ranges for time to enhancement, time to peak enhancement, and peak enhancement intensity were 14 s, 9~14 s; 22 s, 21~26 s~ 83 dB, 55~92 dB, respectively. Conchtsions Rim enhancement pattern on contrast-enhanced sonography seems to be highly suggestive for the diagnosis of pulmonary tuberculomas and parameters derived from time intensity curve seems having limited diagnostic value for pulmonary tuberculomas.
