摘  要:目的运用声辐射力脉冲成像技术(ARFI)对乳腺病灶进行弹性定量研究,探讨该技术在乳腺良恶性病灶中的诊断价值。方法对145例女性患者共151个病灶运用声触诊组织定量技术(VTQ)测量病灶内部区域、边缘区域、对侧同层腺体的剪切波速度(SWV),以病理结果为金标准,构建ROC曲线,判断VTQ技术对乳腺良恶性肿块的诊断价值。结果以乳腺病灶内部区域的SWV值、边缘区域的SWV值、内部区域与对侧同层腺体的SWV比值、边缘区域与对侧同层腺体的SWV比值所构建的曲线下面积分别为0.931、0.925、0.955、0.955,四者间无统计学差异(P>0.05)。四者诊断乳腺病的灶准确性、特异性无差异(P>0.05),而内部区域与对侧同层腺体的SWV比值的诊断敏感性较低(P<0.05)。结论 ARFI技术对乳腺良恶性病灶的诊断价值较高,病灶的内部区域及边缘区域的SWV值可作为有效的诊断参数。Objective To study the diagnostic performance of ARFI technology on the differentiating between benign and malignant breast masses.Methods The values of shear wave velocities of 151 breast lesions were obtained by using ARFI technology,including the internal and marginal areas of target lesion and contralateral mammary gland. The receiver operative curve is used to evaluate the diagnostic performance of different technology.P<0.05 was considered statistically significant in our study.Results There was no statistically differences among the AUC of internal value,marginal value,internal value ratio and marginal value ratio(0.931 vs 0.925 vs 0.955 vs 0.955,P>0.05). The sensitivity of 'internal value ratio' was lower than the others,the specificity and accuracy was close to each other among them.Conclusions Both of the diagnostic parameters including the internal tissue SWV and the marginal tissue SWV have excellent performance in the diagnosis of breast lesions.
