胶原酶联合针刀椎管外闭合松解治疗巨大型腰椎间盘突出症临床研究

摘　　要：目的：探讨胶原酶化学溶解术配合针刀椎管外松解治疗巨大型腰椎间盘突出症的疗效。方法：2008年8月-2011年8月治疗巨大型腰椎间盘突出症62例，利用随机数字表法，将入组患者随机分为治疗组和对照组。其中治疗组32例，男17例，女15例；年龄32～65岁，平均（44．5±4．1）岁；病程6～38个月，平均（18．8±8．3）个月，治疗前joA下腰痛评分（8．8438±4．4149）分；对照组30例，男15例，女15例；年龄30～67岁，平均（43．6±4．5）岁，病程7～40个月，平均（19．0±8．9）个月，治疗前ioA下腰痛评分（9．9±3．9771）分，两组基础资料经统计未见明显异常。治疗组给予胶原酶化学溶解配合针刀椎管外松解治疗，对照组给予单纯胶原酶化学溶解治疗，分别于治疗后1个月和3个月对两组患者进行随访，根据改良joA下腰痛评分标准（29分），对比两组患者的评分进行比较分析。结果：①治疗1个月后j0A下腰痛评分治疗组（17．9375±4．7583）分，对照组为（15．2333±4．1992）分，治疗组明显优于对照组（P〈0．05）；②治疗3个月后，joA下腰痛评分治疗组为（25．1875±2．9560）分，对照组为（18．2667±4．9055）分，治疗组明显优于对照组（P〈0．01）。结论：采用胶原酶髓核溶解术联合针刀椎管外闭合松解治疗巨大型腰椎间盘突出症与单纯胶原酶髓核溶解术相比可迅速缓解症状，远期疗效稳定。Objective ： To explore the curative efficacy of collagenase chemical dissolution technique combined with acupotomology spinal canal release treating huge lumbar disc herniation. Methods：Form August 2008 to August 2011,62 patients accepted treatment of lumbar disc herniation were selected. Using the random number table,enrolled patients were randomly divided into a treatment group and a control group. In treatment group＇s 32 eases, male 17, female 15, aged 32 -65 years old, mean （44.5 ±4. 1） years ohl;course was 6- 38 months,average （18.8 ±8.3 ） months, before treatment,joA low back pain score was 8. 8438 ±4. 4149. In control group＇s 30 patients,15 males and 15 females,aged 30 -67 years old,average （43.6 ±4.5） years old;duration was 7 -40 months,with an average of （ 19.0 ±8.9 ） months; before treatment, the lumbago former joA score was 9.9 ±3. 9771. The basic data of two sets by the statistics had no obvious abnormalities. The treatment group was treated with collagenase chemical dissolution and the acupotomology spinal canal treatment,and the control group was given a simple collagenase chemical dissolution treatment. Followed - up, respectively, at 1 month and 3 months alter treatment,the two groups of patients＇joA low back pain score （29 points） was used to make the comparative analysis. Results ：（1）1 month later,joA of low back pain score in treatment group was 17. 9375 ±4. 7583 ,and in the control group it was 15. 2333 ±4. 1992. The treatment group was significantly better than the control group （ P 〈0. 05）. （2）3 months after treatment,joA of low back pain ratings of treatment group was 25. 1875 ± 2. 9560,and of control group it was 18. 2667 ± 4. 9055. The treatment group was significantly better than the control group （P 〈0. 01 ）. Conclusion： The using the collagenase nucleoplasty chemonucleolysis combined with aeupotomology spinal canal closed loose for huge lmnbar disc herniation can quickly relieve symptoms,and the long -term efficacy is sta
