摘 要:目的探讨白介素-18(IL-18)、白介素-8(IL-8)、干扰素-γ(IFN-γ)在围绝经期综合征患者血清中的变化及临床意义。方法采集50例围绝经期综合征患者(实验组)治疗前、治疗3月后及50例处于绝经期的健康女性(对照组)血清,采用酶联免疫吸附法(ELISA)测定血清IL-18、IL-8、IFN-γ水平,通过放射免疫分析法测定血清雌二醇(E2)、促卵泡刺激素(FSH)水平,对治疗前实验组血清E2、FSH与IL-18、IL-8、IFN-γ相关性进行分析,并对实验组治疗前后进行改良Kupperman评分。结果治疗前实验组血清IL-18、IL-8、IFN-γ、FSH水平明显高于对照组,而血清E2水平低于对照组(P<0.01)。治疗前实验组血清E2与IL-18、IL-8、IFN-γ呈负相关(P<0.05),FSH与IL-18、IL-8、IFN-γ呈正相关性(P<0.05)。治疗3月后实验组改良Kupperman评分及血清IL-18、IL-8、IFN-γ水平较治疗前明显降低(P<0.01)。结论 IL-18、IL-8、IFN-γ可能参与围绝经期综合征发病的免疫过程,血清IL-18、IL-8、IFN-γ水平可作为病情判断及疗效的观察指标。Objective To explore the changes and clinical significance of interleukin- 18 (IL- 18), interleu- kin - 8 ( IL - 8 ) and interferon - 7 ( IFN - γ) in serum of patients with perimenopansal syndrome. Methods The serum of 50 patients with perimenopausal syndrome ( experimental group ) before and 3 months alter treatment as well as 50 healthy women in menopause (control group ) was collected. Serum levels of IL - 18, IL - 8 and IFN - γ were detected by ELISA. Serum estradiolum (E2) and follicle - stimulating hormone (FSH) levels were also measured using radio immunoassay. The correlation of between E2 and FSH and IL - 18, IL - 8 and IFN - γ was analyzed in patients with perimenopausal syndrome before treatment. In addition, the patients in experimental group were evaluated by improved Kupperman score. Resldts Serum levels of IL- 18, IL- 8, IFN - γ and FSH in experimental group before treatment were significantly higher than those of control group ( P 〈 0.01). Serum E2 level was decreased in experimental group before treatment as compared to control group ( P 〈 0. 01). There was negative correlation between E2 and IL - 18, IL - 8 and IFN - γ ( P 〈 0. 05), whereas there was positive correlation between FSH and IL - 18, IL - 10 and IFN - γ in experimental group before treatment ( P 〈 0.05 ). The improved Kupperman scores and serum levels of IL - 18, IL - 8 and IFN - 7 were significantly reduced in experimental group before treatment when compared with after treatment / P 〈 0.01). Conclusion IL- 18, IL-8 and IFN- γ may participate in the immune process of perimenopausal syndrome pathogenesis. Serum levels of IL - 18, IL- 8 and IFN - γ act as novel indicators for pathogenetic condition judgement and curative effectiveness of perimenopausal syndrome.
