经阴道超声检测卵黄囊对早期自然流产结局的价值

摘　　要：目的探讨经阴道超声检测卵黄囊形态大小对早期自然流产结局的预测价值。方法早孕并行阴道超声检查的孕妇1341例，其中自然流产住院患者533例（先兆流产250例，难免流产283例），正常妊娠808例。阴道超声检测卵黄囊形态大小，并追踪妊娠结局。结果孕6～10周在胚胎存在的情况下，卵黄囊缺如或消失140例均妊娠失败；卵黄囊变形11例、卵黄囊壁回声强9例及卵黄囊过小16例，均妊娠失败；33例卵黄囊过大，部分能继续妊娠。结论卵黄囊缺如、消失、过小、变形或壁回声强，妊娠结局不良。卵黄囊过大可适当保胎，但需要观察胚胎发育。Objective To explore the value of yolk sac size and shape at transvaginal sonography to predict pregnancy outcome in the early spontaneous abortion. Methods There were 1 341 cases of first - trimester pregnant patients performed with transvaginal scaning. The yolk sac size and shape were measured by transvaginal sonography. The pregnancy outcome was observed. There were 533 cases of spontaneous abortion inpatients, including 250 cases of threatened abortion patients and 283 cases of inevitable abortion patients, and 808 cases of normal pregnancy patients. Results There were 16 cases of patients with yolk sac smaller than normal, all of them failed pregnancy. For 140 cases of patients at six to ten weeks gestation, in the condition of fetal pole existed, absence of yolk sac or disappeared yolk sac was correlated with failure of pregnancy. There were 11 cases with abnormal yolk sac shape and 9 cases with hyperechogenic yolk sac, all of them had abnormal outcome. There were 33 cases with yolk sac greater than normal, some of them can continue to pregnancy. Conclusion Absence of yolk sac, disappeared, smaller than normal, abnormal shape or hyperechogenic are predictors of poor pregnancy outcome during the first 12 weeks. Patients with yolk sac greater than normal can attempt to prevent miscarriage, but they should be required monitoring.
